As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible. Vessel did not have charts for intended voyage.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Port navigation light was blocked and failed to provide ark of visibility.

Each vessel must be equipped with at least two operable portable battery lights. Vessel was missing 1 flashlight.

If a State issues validation stickers, its numbering system must contain the requirements that stickers must be displayed within 6 inches of the number and the stickers must meet the requirements in paragraphs (b) and (c) of this section. Missing current state registration sticker.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. O/O did not have current proof of chemical testing enrollment

Rails must be permanently installed except that the following rails may be removable; (1) Rails in way of embarkation stations and boarding locations. missing one of the embarkation courses

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. marking wasn't legible

When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. ground strap missing

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: (a) Operation of the main propulsion machinery both ahead and astern. The outboard engine was not available for testing due to a suspected electrical problem.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: (i) Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. The annual maintenance was not conducted for the portable fire extinguishers by the owner or qualified third party.

The manning requirements for a particular vessel are determined by the OCMI after consideration of the applicable laws, the regulations in this part, and all other factors involved, such as: emergency situations, size and type of vessel, installed equipment, etc. Crew CPR/First Aid Certification expired. 46 CFR 15.501
