The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that any non-conformity is reported, with its possible cause, if known. PSCO’s observed two inoperable fire pumps and insufficient quantity of water to the fire main from emergency fire pump. PSCO’s observed fire doors that couldn’t self-close and hatchways in position 2 that weren’t weathertight. An ISM audit is required prior to departure. 74 SOLAS (20) IX/3.1 ISM Code Part A/10.1 ISM Code Part A/10.1

The required fire pumps shall be capable of delivering for fire-fighting purposes a quantity of water, at the pressure specified in paragraph 4. PSCO’s observed the required fire pumps were unable to deliver the quantity of water at the pressure specified in paragraph 4. The water pressure appeared visibly low, and the vessel’s crew was unable to provide a means of testing to verify adequate water pressure was being delivered. 74 SOLAS (97 Cons.) CH. II-2 Reg 4.2.1.2

Fire-extinguishing appliances shall be kept in good order and be available for immediate use at all times. A condition of class was issued to allow the vessel to utilize the general service and ballast pumps for firefighting purposes due to the vessel having two inoperable fire main pumps. PSCOs observed the valve responsible for directing the water to the fire main was stuck in position, disabling the delivery of water. 74SOLAS (97Cons.) CH II-2 Reg 21

At positions 1 and 2 the height above the deck of hatchway coamings fitted with weathertight hatch covers of steel or other equivalent material shall be fitted withgaskets and clamping devices that shall be at as specified in Regulation 15 (1). PSCO’s observed the hatchway between the emergency steering room and the main deck was not weathertight and allowed light to shine though when sealed. ICLL 66/88 Annex I/Reg 16(1)

… Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self-closing. PSCOs observed the door between the Engine Control Room and Engine Room was unable to self-close. PSCO also, observed the door between the main deck and engine room was also unable to self-close, compromising the required fire boundary. 74 SOLAS (97Cons.) CH II-2 Reg 47.1

All external openings leading to compartments assumed intact in the damage analysis, which are below the final damage waterline, are required to be watertight. PSCO’s observed the door leading down to forward CO2 room was not able to be sealed watertight due gasket excessively worn. The inability for the door to properly seal compromised the weathertight integrity 74 SOLAS (97Cons.) Reg 25-10.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or person on deck. The shutoff valve from the section side of the diesel oil purifier does not work by remote activation; however, the diesel oil purifier is not in use. The suction valve will remain out of service and tagged. 74SOLAS(20)I/11; Code 50ac

Class A fire doors shall be possible to be opened and closed from each side of the bulkhead by one person only. The door extending from the main space to the ECR did not open as intended. 74SOLAS(97)II-2/30.3; Code 10c issued.

2 lifebuoys shall be capable of quick release from the navigation bridge. The quick release lifebuoys are installed in a position where they will fall on the deck of the ship upon activation. 74SOLAS(97) III/ 7.1.3

A HAZMAT employee must recieve the training required by this subpart at least once every 3 years. The 2nd officer's HAZMAT training has expired beyond 3 years. 49 CFR 172.704

No person may use transfer pipe system that has not been tested to 1.5 times the maximum allowable working pressure on an annual basis. Vessel has gone over a year without a bunkering piping test.

Each vessel must have a warning statement of conditions on the manuevering information fact sheet. The warning statement is not displayed in the wheelhouse. 33 CFR 164.35

After any survey of the ship has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without sanction of the administration. The second means of starting the EDG, via spring, was inoperable. Vessel will swap batteries for second means of starting the EDG until repairs have been made. SOLAS (2014) CH I/Reg. 11(b).

A self contained energy source of electrical power shall be provided. The emergency source of power onboard the vessel is currently inoperable. 74 SOLAS (1997) CH II-1/REG 43.1.1

2120 - Disc vharge of food wastes shall be comminuted or ground and shall be capable of passingb through a screen with opening no greater than 25MM. Vessel is discharging ungrounded food wastge into the sea. MARPOL ANNEX V/4.2 17bc

1430 - Auxiliary Engines - Each emergency generating set arranged to be automatically started shall be equipped with starting devices approved by the administration with a stored energy capacity of at least three consecutive starts. A second means shall be provided with an additional three consecutive starts. 74 SOLAS 2004 CONS CH II-1/REG 44

0740 - Pumps - In Cargo Ships of 2000 gross tonnage and upwards, if a fire in any one compartment could put all the pumps out of action there shall be an alternative means consisting of a fixed independently driven emergency pump which shall be capable of supplying two jets of water. 74 SOLAS 1997 CONS CH II-1/REG 4.3.3.2

0725 - Fixes Fire Extinguishing installation - Maintenance shall be carried out based on the guidelines developed by the organization. Vessel's Fixed CO2 Firefighting System is out of date on servicing. 74 SOLAS 2014 CONS CHII-2/REG14.2.2

1199 - Other (Cargo) - The Cargo Securing Manual shall be approved by the administration in accordance with IMO MSC.1/CIRC. 1353/ REV. 1. The cargo securing manual is not approved by the Administration. 74 SOLAS 2014 CONS CHVI/REG 5.6

2550 - Maintenance of Ship & Equipment - Recommend external ISM Audit after objective evidence was found for lack of maintenance to vessel's systems and equipment, to include vessel's emergency generator, emergency pump, and CO2 fixed firefighting system.

The ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship remains fit to proceed to sea without danger to the ship or persons onboard. Vessel lost control of the main engine due to the loss of electrical power.
