At each inspection the steering systems shall be tested and inspected to determine they are in a suitable condition. The hydraulic steering lines under the main forward console were leaking oil.

Vapor Barriers must be provided where insulation of any type is used ins paces where flammable and combustible liquids or vapors are present, such as machinery spaces. The forward machinery room bulkhead's through cable run insulation was found to be was cut away, allowing free communication between two spaces. Deficiency rectified.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. The guard on the forward starboard machinery room light fixture was found damaged, compromising the protection capability. Deficiency rectified.

The construction of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull; repair made to port bow of vessel following collision is unsatisfactory for consideration as permanent repair. The small passenger vessel is authorized to transit from Houma, LA directly to Lafayette, LA for repairs. The vessel will remain in Lafayette until hull repair is complete, pending clearance of deficiency by MSD Lafayette. Vessel is not authorized carriage of passengers until acceptable repair is completed. Contact MSD Lafayette for repair process oversight.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repair or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement. Vessel is to transit directly to Fourchon Shorebase LLC dock to effect permanent repairs. Upon arrival, vessel is to contact MSU Houma Command Duty Officer at [phone withheld]. 46 CFR 176.700(a)

Unfired pressure vessel relief valves were not inspected within the last calender year. Remove and test valves and provide documentation.

Prior to operation, vessel is required to undergo hull bottom and appendages inspection to the satisfaction of attending Coast Guard Marine Inspector.

Emergency Escape hatch required the insertion of a metal bar to open hatch from engine room, Gen room, and Steering Room. The Freeman style hatch was not installed with the approved emergency escape handle. Install approved emergency escape handle or replace hatch.

Provide approved bow thruster fluid power piping system plans.
