The forward machinery room bulkhead's through cable run insulation was found to be was cut away, allowing free communication between two spaces. Deficiency rectified.

The guard on the forward starboard machinery room light fixture was found damaged, compromising the protection capability. Deficiency rectified.

The construction of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull; repair made to port bow of vessel following collision is unsatisfactory for consideration as permanent repair. The small passenger vessel is authorized to transit from Houma, LA directly to Lafayette, LA for repairs. The vessel will remain in Lafayette until hull repair is complete, pending clearance of deficiency by MSD Lafayette. Vessel is not authorized carriage of passengers until acceptable repair is completed. Contact MSD Lafayette for repair process oversight.

Repair or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement.
