Electrical equipment on a vessel must be installed and maintained to: (c) Minimize accidental personnel contact with energized parts. 120v engine room electrical panel was missing a blank plate.

The master of the vessel shall ensure that: (c) The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Monthly tests are not being logged.

ALL ELECTRICAL INSTALLATIONS SHALL BE TO THE SATISFACTION OF OCMI. A POWER PANEL IN THE ENGINE ROOM HAS AN OPEN EXOSED CIRCUIT BREAKER SLOT. INSTALL A SLOT COVER FOR EXPOSED OPENING.

ALL ELECTICAL INSTALLATIONS SHALL BE TO THE SATISFACTION OF OCMI. AN OVERHEAD J-BOX IN THE SWTEERING SPACE HAS AN EXPOSED KNOCK-OUT HOLE. INSTALL KNOCK-OUT PLUG IN EXPOSED HOLE.

THE INSTALLATION OF PORTABLE EXTINGUISHERS MUST BE TO THE SATISFACTION OF THE [name withheld] LOCATED IN THE ENGINE ROOM AND OUTSIDE THE SPACE WTD NEED TO BE INSTALLED WITH APPROVED BRACKET #435793.

AN ADULT LIFE JACKET MUST BE PROVIDED FOR EACH PERSON CARRIED ON BOARD AS PER COI. VESSEL HAS 56 SERVICEALBE LIFE JACKETS AND REQUIRES 57. PROVIDE REQUIRED LIFE JACKET AND VESSEL IS RESTRICTED TO 56 ADULT PASSENGERS UNTIL REPLACED.

All portlights which are permitted in a vessel’s sides on vessels operating on exposed or partially-protected waters, whether of the opening or fixed type, shall be fitted with dead covers. (46 CFR 178.40-1(c)-1969) Provide dead covers for six (6) portlights in the forward space, readily accessible for use in case of emergency. The vessel is restricted to the waters of the Chicago River, west of the Chicago lock during the winter season (October 1 – April 15) and to the waters of Lake Michigan west of the outer Chicago breakwalls and north of a track line established between the dome of the Adler Planetarium and the outer breakwall south end light 19980 during the summer season (April 16 – September 30).

Vessels operator insalled a Hot Water Heater and Hand drier without Coast Guard approval or submission of plans prior to installation. Tag out and disconnect units and prior to placing in service submit plans for review and approval to the Marine Safety Center(MSC)

Vessels air receiver could not be proven to meet ASME U or UM. Provide onboard an air receiver that meets these standards or one that bears a Coast Guard Propeller stamp.

Vessels PA was removed during renovations. Prior to carrying passengers install pa system and prove proper operation.

Watertight door in the forward bulkhead of the engineroom is not properly marked. Properly label with 1 inch letters watertight door keep closed on both sides of door.

During testing of Fixed Bilge system the suction check valve in the engineroom was found fracture. Renew and prove proper operation. Also, the forepeak compartment bilge suction is missing the pick up strainer. provide and prove proper operation.

Emergency proceedures where not posted onboard. Post in areas of vessel for crews to reference in an emergency.

Polarity was found reversed causing outlets to not have proper overcurrent protection under the console and in the forward compartment. Properly wire and prove proper operation.

Attach proper terminal lugs to the conductor ends and make proper connections to the terminal board in the ground-detection meter enclosure. 46CFR 183.05-50

Repair ground-fault in pilot house lighting circuit.

Repair battery terminal connection on battery bank located behind generator. Connection is under high strain and half of the copper conductor strands are broken.

Replace charred shore power receptacle on vessel

Repair high level bilge alarm for #4 VOID

Replace work vests for crew w/ type approved for use as work vests IAW 46CFR160.053 or 160.077 as req by 46CFR180.72(c) & (d).

Vsl hull is carrying current when vsl is connected to shore power. Provide isolation xfmr or other acceptable means to prevent ship's hull from carrying current while on shore power. 46CFR 183.370

Replace circuit breaker for shore tie in engine room. Shore tie cable is 10 AWG & is spliced to 6 AWG in the E/R distribution panel & is protected by a 100 amp circuit breaker. Circuit breaker should be sized to provide protection for the 10 AWG cable at approx 30 amperes.

Provide proper sized over-current protection for conductors supplying power to the LED ammeter installed at the engine room switchboard. Wire is 10 or 12 AWG and is currently protected by a 90 AMP circuit breaker which is far too large. Over-current protection needs to be sized to protect the conductor at either 20 or 30 amperes depending on the wire size.

Deficiency is for the engine alternators. Alternator windings are full of oil and must be cleaned. 46CFR 176.830

Vsl approved one line diagram does not match what is on the vsl. Resubmit 120V one line diagram. Drawing must indicate circuit breakers for shore-tie, generator power & the main circuit breaker for the E/R distribution panel. Also, drawing must indicate where the shore tie conductors are spliced & change from 10AWG to 6 AWG conductor. 46CFR 177.202(4) & Part 183

Generator lost power due to fuel pump failure.

Install and demonstrate proper operation of service generator Ammeter iaw 46 CFR 183.10-5 (old T).

Ensure watertight tankroom bulkhead is watertight where wire passes through IAW 46 CFR 179.320

Ensure pull line for EPIRB activation is replaced because of wastage IAW 46 CFR 180.64.

Ensure wiring (12 guage) matches wiring (10 guage) on vessel's electrical plans IAW 46 CFR 177.202 (b)(4).

Prior to carrying passengers, demonstrate proper operation of engine room bilge high level alarm and float switch.

Prior to 19 Oct 07 provide catch chains or other securing devices on all hatch covers exposed to the weather (aft deck).

Prior to 19 Oct 07 properly repair deadlight cover, womens head, by replacing missing dog.

Provide proper insulation of MDE's IAW 46 CFR 182.430

Replace dry routed life rings (2 total) with approved type. IAW 46 CFR 180.70

Conduct permanent repairs to stbd shell plating in holding tank compartment. Damage is located approx between the 4th and 7th frame 40 inches above the turn of the bilge.

PRIOR TO CARRYING PASSENGERS FOR HIRE REPLACE THE PORT SHAFT STERNTUBE CUTLASS BEARING TO THE SATISFACTION OF THE COGNIZANT OCMI.

Vessel is required to stay within 01 mile of shore, Chicago Harbor to planetarium and within the Chicago Harbor/Monroe Harbor during periods of reduced visibility, until the magnetic compass is repaired and a new deviation table is completed.

Renew life floats on vessel. Vessel's route is restricted to within one mile from shore untin requirement is cleared.

MAKE REPAIRS TO O2 PORTLITES FORWARD ON PORTSIDE OF VESSEL. ONE REQUIRES REINSTALLATION OF COVER, ONE REQUIRES REINSTALLATION OF SECURING DEVICE (DOG).

PROVIDE GREAT LAKES AGREEMENT RADIOTELEPHONE CERTIFICATE. TO BE COMPLETED PRIOR TO CARRIAGE OF PASSENGERS ON LAKE MICHIGAN.

PROVIDE CLASS "C" EPIRB. MUST BE COMPLETED PRIOR TO CARRIAGE OF PASSENGERS ON LAKE MICHIGAN.

PROVIDE WOODEN PLANK TO ISOLATE THE GENERATOR REMOTE SHUTOFF VALVE FROM THEFUEL TANK SPACE.

PROVIDE REFLECTION TAPE FOR 3 ADULT LIFEJACKETS AND PROVIDE 3 ADDITIONAL JACKETS. VESSEL IS LIMITED TO THE CARRIAGE OF 65 TOTAL PERSONS UNTIL COMPELTED.

CLEAR THE AFTER HATCH COAMING DRAIN HOLE.

PROVIDE MEANS TO SECURE REACH ROD TO MAIN FUEL TANK VALVE HANDLES TO PREVENT REACH ROD FROM DISENGAGING FROM VALVE HANDLE.

PROVIDE A HAND RELIEVING DEVICE FOR THE AIR COMPRESSOR RELIEF VALVE.

PROVIDE SUFFICIENT DISCHARGE HOSE FOR THE HAND BILGE PUMP.

REPAIR PREHEAT GLOW PLUGS FOR THE GENERATOR (ONLY 1 OF 3 IS WORKING).

CREDIT DRYDOCK DUE 02NOV91. CONTACT USCG PRIOR TO THIS DATE TO SCHEDULE THIS INSPECTION.

REPLACE 2 PLASTIC THRU HULS WITH METALLIC ONES WELDED ON BOTH SIDES.

REPLACE O/B DRAIN HOSES WITH WIRE REINFORCED HOSE AND FIT A VALVE AT THE SIDESHELL.

PROVIDE REMOTE OPERATION FOR THE GENERATOR FUEL SUPPLY SHUTOFF VALVE (OPERATED FROM OUTSIDE FUEL TANK SPACE).

SEAL/MAKE WATERTIGHT THE STBD AFTER HATCH COAMING TO 12" HIGH.

WELD/MAKE WATERTIGHT THE AFTER BULKHEAD OF THE MIDSHIP HOLD.

RENEW THE GASKET IN THE COLLISION BULKHEAD HATCH TO MAKE IT WATERTIGHT.

PROVE OPERATION OF STEERING WHEN REASSEMBLED.

CREDIT DRYDOCK DUE NO LATER THAN 02 MAY 1990.

VESSEL IS OVERDUE FOR DRYDOCK. DRYDOCK THE VESSEL AS SOON AS POSSIBLE BUT NOT LATER THAN 21MAR90.

INSTALL HEAT SHIELD FOR GENERATOR FUEL FILTER.

PROVIDE NAMEPLATE FOR AC GENERATOR.

REPLACE LIGHT IN LAZARETTE AND INSTALL GUARD AROUND LIGHT IN FUEL TANK SPACE.

RENEW GASKETS ON ALL PORT LIGHTS.

PROVIDE ELECTRIC ONE LINE DIAGRAM.

INSTALL FENCING AROUND RAILS PRIOR TO CARRYING PASSENGERS.

PROVIDE L/F OR B/A FOR 50% OF TOTAL PERSONS CARRIED STENCILED WITH VESSEL NAME AND CAPACITY, ATTACHED VIA A 100' PAINTER AND APPROPRIATE FLOATFREE LINK, FLOATING ELECTRIC WATERLIGHT AND IF LIFEFLOATS ARE USED, THEN TWO 4' OARS MUST BE LASHED TO LIFEFLOAT AND STENCILED WITH VESSEL NAME PRIOR TO

PROVIDE REQUIRED NUMBER OF P.F.D.'S FOR TOTAL NUMBER OF PEROSNS ALLOWED BY CERTIFICATE OF INSPECTION.

REPLACE EXISTING MACHINERY SPACE DOOR WITH A CLASS I (HINGED) WATERTIGHT DOOR WITH A QUICK ACTION CLOSING DEVICE OPERATIVE FROM BOTH SIDES OF THE DOOR DESIGNED TO WITHSTAND A HEAD OF WATER EQUIVALENT TO 10 FEET. ALSO, AN INDICATOR IN THE PILOTHOUSE MUST SHOW WHETHER THE DOOR IS OPEN OR CLOSED

STENCIL VESSEL NAME ON EPIRB AND CONDUCT OPERATIONAL TEST.

PROVIDE CERTIFICATE OF INSPECTION, FCC LICENSE, GREAT LAKES AGREEMENT AND PFD INSTRUCTIONS.

REPAIR OR REPLACE RING BUOY AND STENCIL VESSEL NAME ON SAME.

COMPLETE INSTALLATION OF FENCING BETWEEN COURSES OF RAILINGS.

TEST RELIEF VALVE FOR AIR RECEIVER.

INSTALL GUARD AROUND PULLY FOR AIR COMPRESSOR ON STARBOARD ENGINE.

PLACE 8 VOLT BATTERIES IN LINED BATTERY BOX.

REMOVE OR PROPERLY WIRE DEAD ENDED WIRES UNDER GUAGE PANEL.

TEST FIRE AND BILGE PUMPS

INSTALL STRAINERS ON ALL BILGE SUCTION LINES PRIOR TO OPS.

PLUG/WELD MANY SMALL HOLES ON STERN IN WAY OF OLD NAME PLATE PRIOR TO OPS.

COMPLETE ATTACHMENT OF BILGE FRAMES TO NEW INSERTS IN FWD HOLE.
