A vessel to which this subchapter applies must meet 46 CFR 181.405 (interconnected fire detection system) if it is operating on a Coastwise or Oceans route. The interconnected fire detection was not interconnected. Vessel is restricted to operating on a Limited Coastwise route until the system is interconnected to the satisfaction of the MI. 46 CFR 175.110 (c)

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Cover for access to fuel shut off valves has large openings in it. Replace access cover to satisfaction of MI. 46 CFR 177.405 (e)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. MI observes improper seal on both port and starboard ventilation closure devices to engine room. Adjust closure devices to the satisfaction of the MI. 46 CFR 182.465 (h)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency, operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include (1) Closing each valve leading to an overboard discharge and removing the handle; (2) Padlocking each valve leading to an overboard discharge in the closed position; or (3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Vessel did not have overboard secured. Operator installed a chain and lock on the overboard valve. 33 CFR 159.7 (c)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI witnessed the port shaft unbalanced while underway and the drip-less shaft seal leaking. 46 CFR 182.100

In addition to meeting § 180.130, each life float and buoyant apparatus must be stowed as required under this section. MI witnessed forward life float secured by bungee cord, oars not lashed to aft life float, and tangled painter that connected the two life floats. 46 CFR 180.137

A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable and made watertight. MI found two pipes in forward void compartment on the starboard side leading to engine room compartment not watertight. 46 CFR 179.320(c)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed. MI found broken hose clamp on crankcase breather flexible hose. 46 CFR 182.200

A hatch cover must be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. MI found engine room compartment and forward void compartment missing securing devices. 46 CRF 179.360

On a vessel of not more than 19.8 meters (65 feet) in length, a battery powered bullhorn may serve as the public address system if audible throughout the accommodation spaces of the vessel during normal operating conditions. The bullhorn's batteries are to be continually maintained at a fully charged level by use of a battery charger or other means acceptable to the cognizant [name withheld] found vessels public address system non-operational. Repair current public address system or supply a battery powered bullhorn. 46 CFR 183.550

A hatch cover must be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. MI found missing cover on starboard anchor line cover. 46 CFR 179.360

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. MI found deteriorated flexible hose connected to the port bilge overboard connection. 46 CFR 182.500

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI found deteriorated electrical box cover and missing cord connectors under operating station. 46 CFR 183.200

Master of vessel needs to have preemployment drug testing and be enrolled in the vessels drug alcohol programs. Provide required documentations.

Vessels is required to have a logbook onboard at all times. Provide logbook and documented required drills. New crew members need to complete drills prior to getting underway.

Vessel is missing #3 2.5 gallon fire buckets with lanyard and marked "FIRE BUCKET". Provide required buckets.

Masthead light is not operable. Light is required to be working onboard vessel. Limit voyages to day light until light is working.

Aspirin is expired onboard vessels. Provide aspirin that is not expired.

Life float lines are deteriorated on some. Replace lines on both Life floats with same type and length of previous.

Engine room has two dead end wires. Remove wires.

Sewage valve for the MSD is not locked. Install a lock and chain to prevent accidental discharge.

FCC Station license not onboard the vessel. Provide required documentation and keep onboard.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The fuel line from the fuel tank to the generator does not have a shutoff valve at the tank.

Vessels on LC voyages are req'd 12 flares (6 hand red & 6 orange smoke, or 12 hand red); vessel has 8 flares on board. Provide enough flares to equal 12 approved type for voyage. Vessel restricted to LBS route.

Vessel req'd Cat1 EPIRB. Battery on EPIRB was expired (09/2021). Provide new batter for EPIRB, or purchase new EPIRB and register. Vessel restiricted to w/in 3m from shore (not to operate on the high seas).

Fuel shut off valves are req'd to be labeled. Labeling on fule shut off valves has faded/non-existant. Label fuel shut off valves.

Properly label PFD stowage for adults and children. Numbers on plaques did not match quantity of PFD's in container. Provide the PFD's to match qty, or re-label.

Provide copy of FCC station License w/new vessel name. Old station license cancelled, provide new info and post on vessel.

A visual and audible alarm must be provided at the operating station. Engine room light for bilge alarm was inoperable. 46 CFR 182.530

Each survial craft on the vessel must be in good working order and ready for immediate use. Vessel's forward life float is water logged and needs replaced. 46 CFR 185.700

Each life float must be fitted with 2 paddles of at least 4ft and lashed to the life float. Both life float paddles need replaced. 46 CFR 180.175(f)(2)

Master did not have Marine Operator Permit. Vessel is restricted to 1000 ft from shore. 47 CFR 80.159

Vessel life floats must be stowed as far as practicable in a position and protected from fire. Vessel's aft life float was sitting on electrical wiring. 46 CFR 180.130(10)

An exterior lighting fixture operating more than 50 volts must comply with UL 595. Provide UL specs for newly installed deck lights. 46 CFR 183.410(d)

Vessel's stern light was inoperable. 46 CFR 183.420

Electrical equipment in spaces containing machinery spaces must be explosion proof. Lights in engine room were missing guards. 46 CFR 183.530

The wires on the foward lights on upper mast, the connection need to made within an enclosure. 46 CFR 183.340(g) 17JUN2019 - due date extended to 21JUN2019.

Verify hydrostatic realease unit for EPIRB servicing date. 46 CFR 185.740

Each survival craft must be visually inspected to ensure readiness for use. Vessel's aft life float needs patch work due to penetrations. 46 CFR 185.702

Number of penetrations in a watertight bulkhead must be minimized and made watertight. Piping running through bulkhead from engine room to aft void were not watertight. 46 CFR 179.320

Vessel must carry first aid kit that is approved under 46 CFR 160.41. Vessel had incorrect first aid kit.

The fire extinguisher bracket located on maindeck foward needs to be replaced and relocated due to deteriation and bracket does not fully open due to location. 46 CFR 185.500(d)

Each portable extinguisher needs to be of the approved type. Pilot house needs 10 B:C extinguisher 46 CFR 185.500

Clean terminal blocks on batteries in engine room.

Clean edges and tighten hatch scuttles to maintain watertight integrity on Engine Room and Lazarette scuttles. 46 CFR 179.360

Replace gasket on forepeak scuttle. 46 CFR 179.360

Wire connections for lights in pilot house must be in an enclosure. 46 CFR 183.340(g) 17JUN2019 - due date extended to 21JUN2019.

Vessel's radar must be installed and be suitable for the intended speed and route of vessel. Vessel's radar was inoperable. 46 CFR 184.404

An exposed hazard, such as gears or rotating machinery must be properly protected by a cover, guard, or rail. Veesel missing guards on rotating belts on engine. 46 CFR 177.960 17JUN2019 - due date extended to 21JUN2019.

Monthly inspections required ny NFPA 10 needs to be conducted. No monthly tests on portable extinguisher were completed. 46 CFR 176.810(b)(1)(ii)

Stardboard side lights on main deck need sockets.

Conduct satisfactory fire and abandon ship drill in the presence of a marine inspector. 46 CFR 185.520

Life float must meet series 160.027 requirements. Life float needs to be serviced.

17 man lifefloat needs to be serviced.

Provide high level bilge alarms (audible and visual) for storage space. Label alarms properly.

Provide servicing report for fixed fire system.

Remove water in steering compartment and perform sea trials under way.

Provide operable fuel shuutoff valves (03).

Provide strainers for all fixed bilge systems.

Post valid COI, Stability letter and addendum.

Program MMSI number into VHF radio.

Provide proof of valid FCC SSL.

Provide proof of valid EPIRB battery.

Mark life floats with passenger count.

Replace retro tape on lifefloats.

Provide fire buckets (03).

Provide watertight flare box.

Mount electric bilge pump in engine room.

Provide two (02) portable lights.
