THE CONSTUCTION OF ALL DOORS AND DOOR FRAMES IN "A" CLASS DIVISIONS, WITH THE MEANS OF SECURING THEM WHEN CLOSED, SHALL PROVIDE RESISTANCE TO FIRE AS WELL AS TO THE PASSAGE OF SMOKE AND FLAME, AS FAR AS PRACTICABLE, EQUIVALENT TO THAT OF THE BULKHEADS IN WHICH THE DOORS ARE SITUATED. SUCH DOORS AND DOOR FRAMES SHALL BE CONSTRUCTED OF STEEL OR OTHER EQUIVALENT MATERIAL. PSCO OBSERVED FIRE DOOR 2.1.06 UNABLE TO SLIDE SHUT DUE TO CLOSING MECHANISM NOT ACTUATING WHEN ACTIVATED. 74 SOLAS 01 II-2/30.2

EVERY CREW MEMEBER WITH ASSIGNED EMERGENCY DUTIES SHALL BE FAMILIAR WITH THESE DUTIES BEFORE THE VOYAGE BEGINS. PSCO OBSERVED LIFEBOAT COMMANDER ON LIFEBOAT 15 UNABLE TO FIND THE FUEL SHUTOFF VALVE AND WAS UNFAMILIAR WITH ON LOAD RELEASE. ADDITIONALLY, LIFEBOAT COMMANDER AND 2ND OFFICER OF LIFEBOAT 17 DID NOT KNOW WHERE THE FUEL SHUTOFF VALVE WAS. 74 SOLAS 20 III/19.2.1

Each section of sprinklers shall include means for giving a visual and audible alarm signal automatically at one or more indicating units whenever any sprinkler comes into operation. Such alarm systems shall be such as to indicate if any fault occurs in the system. During the testing of section valves 18 and 29, indication on bridge for the intrusion alarm and flow meter displayed a different main vertical zone (MVZ) than what is marked at each section valve locally, both section valves are marked to supply MVZ 07. Section valve 29 intrusion alarm indicates MVZ 06 and the flow alarm indicates MVZ 01. Section valve 18 intrusion alarm indicates MVZ 05. 74 SOLAS (01) II-2/12.1.2. - Code 07109 - Action 40ac.

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of their movement. During testing, hinged fire door, 0A.4.36, in crew galley did not latch. 74 SOLAS (01) II-2/30.4.2. – Code 07105 – Action 10c

The activation of any detector or manually operated call point shall initiate a visual and audible fire signal at the control panel and indicating units. If the signals have not received attention in 2 min an audible alarm shall be automatically sounded throughout the crew accommodation and service spaces, control stations, and machinery spaces of category A. Manual call point in the forward vacuum collection space was found not operational when tested. 74 SOLAS (01) II-2/13.1.4. – Code 07106 – Action 10c

Means of escape shall be marked by lighting or photoluminescent strip indicators placed not more than 0.3 meters above deck to enable passengers to identify all routes of escape and identify the escape exits.
