An inspection is required before the issuance of a Certificate of Inspection (COI). Such an inspection for certification is not made until after receipt of the application for inspection required by 176.105. Vessel's COI is revoked. Submit an application for inspection for a new COI once forward collar is repaired and vessel is ready for inspection. Credit dry docking will be given once satisfactory sponson collar repairs are made. 46 CFR 176.400(a)

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. The blower did not automatically shut down when the fire boy was simulated as deployed. Ensure that the blower will automatically shut down when the fire boy deploys. 46 CFR 181.420(a)(3)

An adult life jacket must be provided for each person carried on board a vessel. Vessel is restricted to 12 persons total due to lack of properly outfitted life jackets. 46CFR 180.71

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position or perform the duties and functions of a position, required by the vessel's COI. Marine employers may form or otherwise use sponsoring organizations, or may use contractors to conduct the random chemical testing programs. Provide satisfactory evidence of a chemical testing program. 46 CFR 16.230(a)

The Assumed Average Weight per person is 185 lb from December 1, 2011 until the AAWPP is first updated pursuant to the provisions of this section. As of the effective date of the first AAWPP update after December 1, 2011, this paragraph (d)(1) will be superseded and cease to be effective. Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). Each portable fire extinguisher on a vessel must be of an approved type. Vessel's portable fire extinguishing equipment has not been serviced in one year. Demonstrate proof of servicing to attending Marine Inspector.

At each inspection of a vessel, the owner or managing operator shall be prepared to conduct tests including the following: Inspection and operation of through hull fittings and sea valves. Demonstrate operation of center through hull fitting valve to attending marine inspector

Each life jacket must be approved in accordance with either 160.002, 160.005, or 160.055 in sub-chapter Q of this chapter, or other standard specified by the Commandant. Found 02 adult life jackets with insufficient retro-reflective tape. Found 08 adult life jackets that have been repaired in a matter not verified by the manufacturer. Reduced passenger count by 10 until an approved life jacket can be provided to each passenger.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. The vessel is missing a suitable patch kit which is specified by Sector Honolulu Work Instruction 07.

Exhaust blower switches must be of the type interloced with the starting switch and the ignition switch so that the blowers are started before the engine starter motor circuit or the engine ignition is engergized. On the vessel found that the engine and the blower are activated separately. Engine can be started without activating blower in normal and fireboy override mode.

Operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license. The operator is responsible for the proper operation of the station. Provide FCC Operators Permit for each master of the vessel.

07116: A space containing machinery powered by, or fuel tanks for, gasoline, must have a ventilation system that complies with this section and consists of: 1) For an enclosed space...(ii) A mechanical exhaust system consisting of at least two ventilation exhaust ducts located at the end of the space opposite from where the supply ducts are fitted. Vessel only has one exhaust duct for gasoline fuel tank space. * This deficiency was initially issued on 20 June 2016. Deficiency was not rectified and company requested special consideration. OCMI responded via letter on 28 March 2018 denying special consideration request, and company had 30 days from receipt of letter to rectify the above mentioned deficiency.
