Each towing vessel must be equipped with navigation
lights, shapes, and sound signals in accordance with the
International Regulations for Prevention of Collisions
at Sea (COLREGS) or 33 CFR part 84 as appropriate to its
area of operation. All three mast lights were missing
bulbs and not operable during inspection.

gap in gasket and allows light to come in when dogged

RPM. Vessels auxiliary generator RPM gauge was
inoperable. Fix RPM Gauge to the satisfaction of MI.

used in lengths of not more than 30 inches; (ii) Is
visible and easily accessible; (iii) Does not penetrate
a watertight bulkhead; (iv) Is fabricated with an inner
tube and a cover of synthetic rubber or other suitable
material reinforced with wire braid; If designed for use
with compression fittings, is fitted with suitable,
corrosion-resistant, or fittings compliant with the SAE
J1475. Port and Stbd main engines have worn hydraulic
hoses coming off of reduction gear, Replace hoses to
comply with SAE J1942 to satisfaction of MI.

The owner or managing operator must implement procedures to identify and mitigate health and safety hazards... Operators utilize pump in the forward space to transfer fuel to deck equipment. During the transfer, a person is in the confined space in order to shut the valve in case of emergency and the closest extinguishers are on the bridge/galley. Review the health and safety plan and identify if any changes should be made to make the evolution safer for the individuals involved. Submit plan to the Coast Guard for review.

power-driven, fixed fire pump, a fire main, and hoses
and nozzles in accordance with paragraphs (a) through
(d) of this section; or a portable pump, and hoses and
nozzles, in accordance with paragraphs (e) and (f) of
this section. Fire hose was found with pinholes in it
during operation. Operator provided vessel with new hose
and attending MI corrected on the spot.
