General. Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Unless otherwise stated in this section, each item of equipment carried, whether required under this section or not, must be secured by lashings, stored in lockers, compartments, brackets, or have equivalent mounting or storage arrangements that do not: (3) Interfere with launching, recovery, or rescue operations; On the stbd 50 person life raft, replace the strap equipment with the same strap on the other two life rafts. For the 6 person life raft, replace the frayed strap with a new one.

(b) Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. MI found both batteries in the port jet room, without covers and with water near the terminals.

A pre-engineered fixed gas fire extinguishing system must:(3) Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. STBD side engine #4 did not secure when fire boy was tested.

Electrical equipment on a vessel must be installed and maintained to: Provide services necessary for safety under normal and emergency conditions. Starboard forward engine room light was extinguished. 46 CFR 183.200(a)

Each survival craft must be secured to the vessel by a painter with a float free link. Identified IBA's hydrostatic release units were not fitted with a shackle from painter to weak link IAW Hammar manufacturer mounting instructions.

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, not more (6 feet), securing the waterlight around the body of the ring life buoy. Identified lanyard has deteriorated. Replace lanyard.

A self priming power driven fire pump must be installed on each vessel of more than 65 feet in length. The vessel reported the engine driven fire pump was damaged and unable to provide pressure to the fire main. The vessel shall maintain a portable fire pump onboard along with the secondary electric fire pump until the primary engine driven pump is repaired. Effect permanent repairs and demonstrate proper operation of the primary fire pump within 5 days. 46 CFR 181.300

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Identified high water alarms in both jet voids were not mounted correctly and were found stowed on transverse frames.

At each inspection for certification and at every other vessel inspection all observed unsafe practices must be corrected. Identified coolant leak on aft engine in Port engine room.

The master of the vessel shall ensure that each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly. Identified EPIRB had not been tested/logged, and hydrostatic release was expired.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition...Fuel is leaking from the fuel return line J1942 flex hose & J1475 hose fitting on top of the port side fuel tank. Properly repair fuel leak. 46 CFR 176.830

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets and the number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. 46 CFR 185.604

The owner or managing operator shall notify the cognizant OCMI as far in advance as possible whenever a vessel is to be hauled out or placed in a drydock or slipway in compliance with § 176.600 or to undergo repairs or alterations affecting the safety of the vessel, together with the nature of any repairs or alterations contemplated. Hull repairs or alternations that affect the safety of the vessel include but are not limited to the replacement, repair, or refastening of planking, plating, or structural members including the repair of cracks...Submit plans to make permanent repairs to two cracks at the aft corners port and stbd bulwarks iwo of the vessels bow at the vessels next dry dock 01/31/2023. 46 CFR 176.665

All cable and wire must: Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support, except that on vessels of not more than 19.8 meters (65 feet) in length, installations in accordance with paragraph 14.h of ABYC E-8 and paragraph 15.h of ABYC E-9 (both incorporated by reference; see 46 CFR 175.600) are acceptable as meeting the requirements of this section...Properly install metal wire supports for all wire and cable runs. Wires in the stbd generator machinery space are supported only with plastic tie wraps. 46 CFR 183.340

Vessel experienced a faulty fuel injector alarm on #3 Main engine. Vessel is required to trouble shoot and make repairs to satisfaction of attending CGMI with in Seven days. Vessel is not restricted as the remaining three engines a capable of safely navigating the vessel and vessel meets the intent of their COI.

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Two mini refrigerators were plugged into a power strip under the concession stand.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire. MI observed various areas in the port engine room that had oil accumulation. Clean bilge and locations surrounding main engine to mitigate fire potential.

A space containing diesel machinery must be fitted with adequate means such as dripproof ventilators, ducts, or louvers, to provide sufficient air for proper operation of main engines and auxiliary engines. Powered ventilation for the port machinery space is producing a fault on the speed control box, causing it to be inoperable. Repair and restore to original condition.

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. COI is currently posted on a ceiling panel in the starboard passageway. Post in a more readily seen location.

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information: date of training and general description of the training topics. Deckhand and naturalist (Tucker and Bree) were not listed under section 5. "Crewmembers Verification of Orientation and Training" of the Vessel Orientation and Training Manual. Conduct training or prove training was conducted on a sister vessel.

Public address system was inoperable.

Battery covers were missing on the port and starboard generator battery banks and the port engine battery bank.

SFP lagging was falling down from the edges around the stbd engine room ventilation shaft.

Stbd engine room ventilation did not close upon testing.

STBD side void #1 bilge suction was inoperable.

Both fuel tanks fitted with plastic fuel gauge tubes and not self-closing valves. Valves must be self-closing and sight gauge must be glass. Replace fuel gauge with appropriate gauge. This was written as a deficiency due no later than 11FEB2021

Repair exhaust leaks on all main engines prior to operating engines.

Starboard aft engine is out of order for engine rebuild. Prove proper operation prior to using engine.

Repair port aft main engine coolant, exhaust and oil leak.

Replace exhaust lagging on all turbos.

Port fwd engine had exhaust leak on inboard side. Exhaust leak was long standing evidenced by exhaust blowing a hole through the exhaust lagging.

Starboard aft main engine has an excessive oil leak, pooling oil on engine and in bilge creating a fire risk. Repair engine and clean bilge of excess oil. Repair and prove proper operation to attending marine inspector prior to using engine.

Starboard aft engine is leaking sea water from wet exhaust system. Repair to original condition and prove operation prior to using engine.

Starboard fwd engine is out of order. Vessel master was not sure how long engine has been out of order. Prove proper operation of engine to attending marine inspector.

Vessel crew drills were not logged as required. Conduct drills and log as required.

Vessel crew was unfamiliar with the fixed fire fighting systems onboard. Ensure vessel crew is knowlegable of fire fighting systems and emergency actions required.

Install all batteries in the battery box or corrosive resistant trays

Vessel shall provide plan review documents and drawings for newly installed fire pump and associated piping for approval by December 1st 20219.

Vessel shall provide Coast Guard Marine Safety Center with information on all added weight with location for tracking of weight changes that could affect the vessel’s Stability Letter by December 1st 2019.

Vessel needs to provide means to engage/operate fire pump locally

Bulkhead piping penetrations for systems with non-metallic piping must have a metallic valve at the bulkhead; renew or replace the plastic sewage valve at the aft bulkhead penetration in the #2 port void.

Fuel fill piping utilizing non-conducting flexible fill pipe sections shall be bonded to protect against static charge; fuel fill flexible sections in the overhead of the port fuel void are not bonded.

Identified the vessel had insufficient power sources for the electrically powered vital systems listed in 182.710(a). Provide proposal/plans to address redundancy requirements.

Stowage containers for life jackets must be marked "lifejackets" as well as the quantity and size in each container (46CFR185.604(f)).

Repair STBD side steering system prior to sailing.

Properly mark location of all children's lifejackets. Remove "rear" from current sign or replace with new one. All underseat locations are to be marked as required.

Properly mark location of all adult lifejackets stowed in main cabin, under fwd cabinet.

Demontrate satisfactory crew safety/emergency drills (M.O.B. and fire).
