For all ships the means of embarkation shall be inspected and maintained for intended purpose. PSCO observed starboard and port embarkation ladders had numerous cracks on the lower steps edges which had been concealed by the crew. 74 SOLAS (2020) II-1/ 3.9.3

PSCO observed sections of missing A-60 insulation in the CO2 room port side forward bulkhead. 74 SOLAS (20) II-2/14.2.1.2.1.1.1

The company and the ship shall comply with the requirements of the ISM Code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. The company should ensure appropriate corrective action is taken. PSCO observed during an expanded exam the following: failure of the company and ship to properly manage failing E/R equipment to include; D/G #1, #2, #3, #4, M/E cooling water systems, M/E high pressure F/O lines. An external ISM audit is required prior to release from the detention. 74SOLAS(20) IX/3.1 ISM Code 10 30abc

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed excessive leakage of M/E jacket water cooling from #4 and #9 cylinders. 74SOLAS(20) I/11(a) 60abc

PSCO observed significant exhaust leak from the D/G #2 exhaust manifold and significant deterioration of M/E exhaust manifold in several locations near the turbo chargers. 74SOLAS(20) I/11(a) 60abc

The arrangements for the storage, distribution and utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons on board. PSCO observed excessive leakage under the L/O drive pump on the front of the #1 D/G, as well as, excessive leaking of oil from sludge pump. 74SOLAS(04) II-2/4.2.3.1 17abc

The carriage of dangerous goods in packaged form shall be in compliance with the relevant provisions of the IMDG. Cargo transport units containing dangerous goods packed in limited quantities shall be placarded and marked according to 3.4.5.5. PSCO observed that container FCIU8773331 located in position 70/12/84 contained limited quantities of dangerous goods but was not placarded accordingly. 74SOLAS(20) VII/3 IMDG Code 5.3.2.4 17c

The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. For this purpose, the following functional requirements shall be met: fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO observed fire hose gaskets dry-rotted and deteriorated at locations next to #1 S/W pump, in upper engine room next to boiler and fire hose station E-28. 74SOLAS(20) II-2/14.1.1 17abc

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed a zip tie securing the cover to the electrical conduit box on the #2 compressor. 74SOLAS(04) II-1/40.1.3 50abc

All access openings in bulkheads at ends of enclosed superstructure shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the un-pierced bulkhead and weather tight when closed. Means for securing these doors weather tight shall consist of gaskets and clamping devices or other equivalent means and shall be permanently attached to the bulkhead. PSCO observed that the door leading to engine room positioned in the superstructure was impeded from closing fully and daylight was observed from the inside. ICLL 12.1 50abc

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and liferaft embarkation deck. For this purpose, the following functional requirements shall be met: escape routes shall be maintained in a safe condition, clear of obstacles. PSCO observed a deep freezer in the provisions space obstructing the escape route to the lifeboat embarkation deck. 74SOLAS(09) II-2/13.1.2 50abc

The means for securing and maintaining weather tightness shall be to the satisfaction of the Administration. The arrangements shall ensure that the tightness can be maintained in any sea condition. PSCO observed significant deterioration of hatch cover in bow thruster space leading to cargo hold No1 and evidence of water intrusion based on corrosion of ventilation duct below hatch cover. ICLL 16(4) 50abc

Suitable handholds shall be provided or a buoyant lifeline shall be becketed around the outside of the lifeboat above the waterline. PSCO observed one portion of the buoyant lifeline on the port side of the port side lifeboat broken and sagging. 74SOLAS(20) III/31.1.1.1 LSA Code 4.4.7.3 50abc

External high-pressure fuel delivery lines between the high-pressure fuel pumps and fuel injectors shall be protected with a jacketed piping system capable of containing fuel from a high-pressure line failure. High pressure fuel lines no. 2,3,5,7,9,10, and 12 were missing double jacketing and were wrapped in insulation, tape, and plastic zip ties. Connections to the fuel delivery system show evidence of fuel leaking onto cylinder assembly. 74 SOLAS (04) II-2/4.2.2.5.2

External high-pressure fuel delivery lines between the high-pressure fuel pumps and fuel injectors shall be protected with a jacketed piping system capable of containing fuel from a high-pressure line failure. High pressure fuel lines no. 2,3,5,7,9,10, and 12 were missing double jacketing and were wrapped in insulation, tape, and plastic zip ties. Connections to the fuel delivery system show evidence of fuel leaking onto cylinder assembly. 74 SOLAS (04) II-2/4.2.2.5.2

The ignitability of combustible materials shall be restricted. PSCO’s discovered, in multiple areas of the engine room hidden in voids, large amounts of oil containers, chemical containers, paint, and oil drums. 74 SOLAS (04) II-2/4.1.3

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. 02 washers in the crew laundry room were spliced together into one cord created an electrical hazard. 74 SOLAS (04) II-1/40.1.3

The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. The technical nature of deficiency 01 provides objective evidence of a serious failure of the implementation of the ISM Code. An internal safety audit and corrective action report is required within three months. 74 SOLAS (20) IX/3.1 ISM Code Part A/10.3

The company should ensure that all personnel involved in the company’s Safety Management System have an adequate understanding of relevant rules, regulations, codes, and guidelines. Log out/tag out procedures are clearly incorporated into the shipboard SMS and were not followed when applying a lock out danger tag on the sanitary space exhaust fan. LO/TO procedural training was conducted following a deficiency issued during a PSC exam on 17JAN2024 which identified failure to implement the ship’s SMS also in regard to LO/TO procedures. 74 SOLAS (20) IX/3.1 ISM Code Part A/6.4

The deficiencies below indicate a serious failure, or lack of effectiveness, of the implementation of the ISM Code. A safety management audit shall be carried out by the Administration or the RO prior to the ship being release from detention. SOLAS (2020 Cons) IX/3.1 ISM Code 10.1

Machinery, associated piping and fittings shall be adequate for the intended service. The fire main piping is holed and has a significant leak at the flange in the engine room. The leaks are located between the two main fire pumps. As such, regardless of which fire pump is running, the piping leaks cannot be isolated. In addition, there is a leak in the bilge eductor pump, which essentially leaks any water pumped from the bilge back into the bilge. The significance of the leaks was sufficient to activate all three bilge alarms in the engine room, which were found to be silenced by the [name withheld] (2020 Cons) II-1/26.1

The Company should establish procedures for key shipboard ops concerning the safety of personnel. The vessel's Log Out/Tag Out procedures, which are clearly incorporated into the shipboard SMS, were not followed during a change of the #3 blower motor for the scavenge air system. The power cable was taped, and while the breaker was found to be in the "open" position, it was not tagged out. The vessel had a previous crew injury related to failure to follow log/out tag/out procedures in November 2023. SOLAS (2020 Cons) II-1/40

Means shall be provided to control leaks of flammable liquids. HFO Purifier was discovered to be leaking excessively (~1 drip/ every few seconds). The body of the purifier equipment was coated in fuel oil and the adjacent drip pans were full. SOLAS (2020 Cons) II-2/4.1.1

Machinery/piping shall be installed and protected as to reduce to a minimum any danger to persons on board with due regard for hot surfaces. Vessel has steam leak on M/E jacket water heater. SOLAS (2020 Cons) II-1/26.1

The company and the ship shall comply with the requirements of the ISM code. The company should establish and maintain procedures for identifying any training which may be required in support of the safety management system and ensure that such training is provided for all personnel concerned. PSCO observed, during an expanded exam, the following: failure of the company and ship to properly follow SMS Fleet Instruction Manual Section B 2.5.4.10. Lock-out / Tag-out (LO-TO) and Hazardous Energy Control during the repair on the L/O purifier heater steam line.An external ISM audit is required prior to next U.S. port after sailing foreign. 74SOLAS(20)IX/3.1 ISM code 6.5

PSCO observed missing components to the F/O system on the #3 A/E. 74SOLAS(04) II-I 26.1

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. PSCO observed excessive leakage of M/E jacket cooling water from cylinder head #9. 74SOLAS(04) II-I 26.1

Means shall be provided to keep the starting air pressure at the required level where internal combustion engines are used for main propulsion. The vessel is designed to provide 12 consecutive starts, upon marine casualty, vessel was determined to only provide for 08 consecutive starts.74 SOLAS (2004) II-1/53.4.5

Whenever an incident occurs to a ship or a defect is discovered...the master or owner of the ship shall report at the earliest opportunity to the administration. PSCO observed a fault alarm on the oil mist detector. Vessel is awaiting parts but was unable to provide that the notification was made to the administration.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. The company should ensure; Appropriate corrective action is taken. PSCO observed, during an expanded PSC Exam, the following: Failure of the company and ship to properly manage failing E/R equipment, to include, A/E #3, boiler, Oily water separator,fresh water ejector pump, M/E Smoke detector, M/E blowers, ballast water system. In addition, PSCO observed local water mist system in "Manual" mode, as well as physically off (ball valves in the closed position) in E/R due to severe leakage in the system when the valves are in the open position. Chief engineer requested parts to repair the system on 14OCT2022, which have not been received. An external ISM audit is required prior to release from detention. 74SOLAS(20) IX/3.1 ISM Code 10

After any survey of the ship under regs 7,8,9 or 10 has been computed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the administration. PSCO observed the fresh water ejector pump with a missing check valve opening. PSCO also found M/E blowers 1,2,3 disconnected and inoperable. 74SOLAS(20) I/11(b)

The arrangements for the storage, distribution and utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons on board. PSCO observed A/E #3 leaking L/O. 74SOLAS(09)II-2/4.2.3.1

Fire fighting systems & appliances shall be kept in good working order and readily available for immediate use. PSCO found Hyper Mist System in manual mode and all manual valves in closed positions. 74 SOLAS (09) II-2 14.2.1.2 Action 30abc

After any survey of the ship under paragraph 1 of this regulation has been completed, no change shall be made in the structure, equipment, systems, fittings, arrangements or material covered by the administration, except the direct replacement of such equipment and fittings. While testing OWS, PSCO found pipe from bilge holding tank to OWS severed and discharging bilge water. MARPOL I/6.4.2 Action 30 abc

Fire detection and fire alarm systems shall be kept in good order so as to ensure their required performance if a fire occurs. PSCO found smoke detector over M/E removed. 74SOLAS(09) II-2 14.2.1.1 Action 30 abc

The machinery, boilers and other pressure vessels associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed significant steam leaks coming from boiler in multiple locations. 74SOLAS(04)II-1/26.1

Fire Fighting Systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed the international shore connection box locked on both port and starboard. 74SOLAS(09) II-2/14.2.1.2

The owner/operator shall install and operate a Ballast Water Management System (BWMS) that has been approved by the Coast Guard under 46CFR part 162. Ballast water treatment system is inoperable and flag dispensation expires while in U.S. waters. 33CFR151.2025

PSCO observed MDE #8 cylinder high pressure fuel line saturated with oil, multiple high pressure fuel lines on the MDE supported by rope lashings, #11 MDE cylinder containment underneath the fuel pump was nearly full and #2 MDG fuel oil piping actually leaking at the valve. Additionally, #12 cylinder hydraulic line on MDE was saturated closest to the pump. 74 SOLAS 20 II-2/4.1.1 17 ac

11101-Each survival craft shall be stowed in a state of continuous readiness so that two crewmembers can carryout preparations for embarkation and launching in less than 5 min. PSCO observed holes on port side lifeboat keel cooler line and engine jacket water reserve empty. Upon further investigation, PSCO also noted that the weekly checks were conducted with no discrepancies noted. 74 SOLAS 20 III / 13.1.3 17 ac

13101- The machinery, boiler, and other pressure vessels associated piping systems and fittings shall be of adequate design and construction adequate for the service for which they are intended. PSCO observed bolts missing from #11 MDE cylinder mancover and active cooling water leak coming from MDE #7 cylinder head. 74 SOLAS (04) II-1 / 26.1

07106- A fixed fire detection and fire alarm system shall be provided in accordance with the provisions of this regulation. Two smoke detectors in the machinery shop were found covered. Additionally, several smoke detector were found damaged in the generators space. 74 SOLAS (20) II-2/ 7.2.1 17ac

09209- All electrical apparatus shall be so constructed and installed as not to cause injury when handled or touched in normal manner. PSCO observed electrical cabinet for V/V remote control system with wooden block spanning the door handles acting as locking device. 74 SOLAS 04 II-1/45.1.3 17ac

13199- After any survey of the ship under regulations 7,8,9,or 10 has been completed no change shall be made in the structural arrangements machinery equipment and other items covered by the survey without the sanction of the Administration. PSCO observed temporary repairs and multiple pinhole leaks on fwd and aft mooring winches. 74 SOLAS 20 I/11(b) 17ac

15109-The company shall establish procedures to ensure the ship is maintained in conformity with the relevant rules and regulations and with additional requirements which may be established by the company. Per the vessel's SMS procedures outlined in QP07, any defects identified by the crew during continuous inspection and everyday workday onboard shall be recorded. Vessel's crew have been noting no discrepancies on weekly / monthly logbook despite the current conditions of the machinery spaces as indicated in deficiencies 1-6. Due to objective evidence the COTP questions the adequacy and overall implementation of the vessel's SMS under the ISM code. An internal audit is required within 30 days to determine whether the ship is operating IAW the ISM code. Provide a copy of the audit report to the USCG when completed. 74 SOLAS 20 IX/3.1 ISM Code 10.1
