Special consideration shall be given to the suitability of any essential component which is not duplicated. PSCO observed the starboard aft actuator was leaking.

Tankers operating with a cargo tank cleaning procedure using crude oil washing shall be fitted with an inert gas system complying with the FSS code. While conducting the operational test of the IGS, the PSCO's found the gasket on the outlet side of blower No.2 leaking and filling the fan room with IG exhaust.
