Stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel did not have FCC Station License. 47 CFR 80.13

Vessel had multiple emergency lights inoperable. Verify all emergency lights are operational. 46 CFR 112.15-1 (c)

The main steering gear and rudder stock must be capable of moving the rudder from 35° on either side to 35° on the other. #2 steering pump in non-follow up is automatically going hard left and remains there even when trying to make steering correction. 46 CFR 58.25-10

Inspections shall include the ability of the crew to effectively carry out abandon-ship and fire drill. Prior to activation from ROS, the following items are required: -Manning review including crew licenses, credentials, endorsements & Fire and abandon ship drills. SOLAS 2020 Ch. III Reg. 19.3 & 46 CFR 199.45(b) 6

Means of escape so that persons onboard can safely and swiftly escape to the lifeboat and liferaft embarkation deck. Additional aids for escape shall be provided as necessary to ensure accessibility, clear marking, and adequate design for emergency situations. Bow escape hatch opening device was not operational and the signage was faded. SOLAS 04 Ch. II-2 Reg. 13.1

Each survival craft shall be stowed in a state of continuous readiness. SRBD Life Boast was not able to be tested due to maintenance/ engine repairs. SOLAS 98 Ch. III Reg 13

Al ships shall have a compass bearing device with a means of correcting heading and bearings to true at all times. Compass swing card was over 40 degrees off. SOLAS 20 Ch. V Reg. 19.2.1.3

The oil record book shall be completed on each occasion any of the following machinery operations take place: bunkering of fuel, collection or disposal of sludge, discharge of bilge water, or failure of oil filtering equipment. The ORB (H) entries do not indicate total contents of the tanks as documented in the 18NOV21, (H) entry. The 19NOV21 (D) entry is missing the information required under D.14; also the entry incorporates disposal of sludge which should have been logged under (C) instead of (D). On 8SEP21 the failure of the oil filtering equipment was logged with no time the system was made operational as of the date of this inspection. Ensure that an appropriately maintained ORB is maintained on board the vessel for inspection at all times. MARPOL (17) I/17

Prove that the vessel’s engines listed on its IAPP certificate have been maintained within the limits of Tier I and the NOx Technical Code. The record of changes to the emission related components have not been kept in neither the engines technical file nor the vessel’s preventative maintenance program. In addition, it was identified that engines S2B0029 had 37476 hours and S2B0029 40009 hours and both were recently overhauled during a Dec20 – Jan21 maintenance period. Neither files were updated with the record of change. MARPOL (17) VI/13.3

All starting arrangements for the emergency generator shall be located in the emergency generator space. The second source of energy for starting the emergency generator are batteries which are located in the main engineering space.

SOLAS 74/78, Chpt.III,Reg. 20.2: Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Vsl's stbd rescue/lifeboat steering gear failed due to damaged teleflex steering cable. Conduct repair IAW manufacturer's procedures and demonstrate proper operations to the attending USCG MI.

All ships 3,000GT and updwards shall in addition to meeting the requirements of paragraph 2.5, have a 3 GHz radar and and a 2nd 9GHz radar or other means to determine and display the range and...which are functionally independent of those referred to in paragraph 2.3.2. Vsl's 3cm radar in inoperable. Repair IAW and demonstrate proper operation to the attending USCG MI.
