In particular, those components which may be subjected to corrosive action of the gases and/ or liquids are to be either constructed of corrosion-resistant material or lined with rubber, glass fibre epoxy resin or other equivalent coating material. Found five gas delivery piping corroded and holed at the stainless steel weld seam located on the fluegas piping delivery gas to the scrubber. The hole is approximately one inch wide by 1/2 inch tall and is leaking water and flue gas into the space. Also found leaking water on the weld seam for water drain piping on the bottom of scrubber. The flue gas corroded piping is permmiting flue gas and water into the space, creating a dangerous atmosphere for personnel when the system is operating.

0715: The function of the fixed fire detection and fire alarms system shall be periodically tested to the satisfaction of the administration. Vessel does not have means/ equipment to produce hot air at the temperature to which the heat detectors are designed to alarm. 74SOLAS(14)II-2/7.3.2, 17C.

46CFR 39.20-1(e) 50c Each vessel vapor connection flange must have a permanently attached. Missing stud on forward port side vapor flange.

46CFR 30.10-25 50c Flame screen means a fitted single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two fitted screens both of corrosion-resistant wire. Flame screen on PV Breaker found to have multiple holes.

0799 Self closing fire doors are not to have any hold back devices. Multiple hold back devices were found on self closing fire doors through out the accommodation. The vessel is to remove the hold back devices to the satisfaction of class.

1820 There was evidence of leakage on the following items: hydraulic leaks on cargo oil valve #304 and 2 wing ballast valves beneath the wing ballast eductors. cargo leakage on cargo valve #306,204, flange inboard of stripping tank valve #27, and #2 cargo pipe flange salt water leakage from flange above #1 wing ballast eductor. two pressure sensors missing. class needs to verify repair and testing

1710 Every oil residue tank listed in the IOPP form B is required to have weekly soundings recorded in the oil record book. the stuffing box drain tanks numbers 1 and 2 are not being recorded as required.

1705 The SOPEP is required to have local contact information for the COTP zone. The vessel's qualified individual (O'Brien) provided the vessel with wrong phone number for San Francisco's COTP in the VRP and did not provide any local infromation in the SOPEP.

2535 Ballast water management plan is missing policy on sediment management and recording. the vessel should be provided with guidance on sediment management and recording of sediment. Address to the satisfaction of the administration and class.
