Each incinerator on a ship constructed on or after 01 January 2000 shall meet the requirements contained in appendix IV to this Annex. Each incinerator subject to this paragraph shall be approved by the Adminstration taking into account the standards developed by the Organization. PSCO observed that the refractory brick in the incinerator was significantly deteriorated.

For tankers of 20,000 tonnes deadweight and upwards constructed on or after 1 July 2002 but before 1 January 2016, te protection of the cargo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the Fire Safety System Code, as adopted by resolution MSC.98(73). The system shall becapable of delivering inert gas with an oxygen content of not more than 5% by volume to the cargo tanks at any required rate of flow. Oxygen analyzer was malfucntioning and unable to provide proper oxygen content information.
