Each BWTS manufactured under CG approval must have a nameplate secured to the BWMSand plainly marked. PSCO observed no approval plate on the system. 46 CFR 162.060-22 50ac

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the enviornment. The various tasks should be defined and assigned to qualified personnel. No written procedures detailing inspections required prior to cranes being operated could be provided. Based on the findings regarding deficiencies #2 and #3, an internal audit of teh SMS procedures are required within 90 days w/focus on crew certificaes and written procedures for key shipboard operations. 74SOLAS(14)IX/3.1 ISM Code 7 Action Taken: 705c

CREW COULD NOT PROVIDE DOCUMENTATION REGARDING MAINTAINING "A" BOUNDRY WHEN UNDERWAY. REQUEST CLAASS CLARIFICATION FOR "A" BOUNDRY SEP[ERATING MAIN MACHINERY ROOM AND STEERING GEAR ROOM.

BEFORE THE SHIP LEAVES PORT AND AT ALL TIMES DURING THE VOYAGE, ALL LIFE-SAVING APPLIANCES SHALL BE IN WORKING ORDER AND READY FOR IMMEDIATE USE. RESCUE BOAT ENGINE WOULD NOT START. VESSELS CREW CLEANED ENGINE IGNITION COIL AND PSCO OBSERVED OPERATIONOAL TEST PRIOR TO DEPARTING VESSEL.
