47 CFR 80.1005 - Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Vessel did not have a bridge-to-bridge certificate on-board. Provide evidence of a completed inspection conducted by a licensed FCC technician to the USCG Marine Inspector.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. Oxygen and Acetylene hoses were severely dry rotted. Replace oxygen and Acetylene hoses to the satisfaction of the USCG Marine Inspector.

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles in accordance with this section. Vessel was equipped with a fire hose repaired with duct tape. Replace fire hose to the satisfaction of the USCG Marine Inspector.

Each operator or agent of a vessel with a capacity of 250 or more barrels of fuel oil, cargo oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154, or each person who arranges for and hires a person to be in charge of a transfer of fuel oil, of a transfer of liquid cargo in bulk, or of cargo-tank cleaning, shall designate, either by name or by position in the crew, the person in charge (PIC) of each transfer to or from the vessel and of each tank-cleaning. Vessel was found to be conducting fuel oil transfers with the dredge OREGON which does not currently comply with required transfer personnel, procedures, equipment, or records. ITV W. L. WILLIAMS is restricted from receiving fuel from the dredge OREGON until the dredge is in compliance with 33 CFR 155 to the Marine Inspectors satisfaction.

Each towing vessel must provide alarms for the auxiliary generator engine low lubricating oil pressure and high cooling water temperature. Could not verify alarms on the starboard generator. Demonstrate alarms functioning properly.46 CFR 143.230 (a)(3)&(4)

Carbon dioxide cylinders forming part of a fixed fire extinguishing system must be retested at least every 12 years. Flexible connections between cylinders and distribution piping of semi-portable or fixed carbon dioxide fire extinguishing systems and discharge hoses in semi-portable carbon dioxide fire extinguishing systems must be replaced or tested at a pressure of 6.9 MPa (1,000 psig). At test pressure, the pressure must not drop at a rate greater than 1.03 MPa (150 psi) per minute for a 2-minute period. The test must be performed when the cylinders are retested. Cylinders must be marked. CO2 system had stamps on cylinders showing the last test was performed more than 12 years ago. Provide documentation attesting to servicing to meet the requirements. 46 CFR 142.240 (a)(2)&(3) 46 CFR 147.60 & 147.65

Fuel system requirements for towing vessels require filters and/or purifiers. Where filters are used, the fuel filters must be replaced in accordance with manufacturer's requirements. Racor filters are installed with clear plastic sight glass without protective metal flame bowls. Plastic covers only meet the manufacturer's specifications when installed with the metal flame bowls in a marine application. Install flame bowls to meet the requirement. 46 CFR 143.255.

A towing vessel of at least 26 feet, but less than 79 feet, in length must carry a minimum of two lifebuoys approved under approval series 160.050 or 160.150. Both Lifebuoys were found to be wasted and need to be replaced. 46 CFR 141.360(a)(2) & (b)(1).

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. Exhaust piping in vicinity of operating station is not insulated or shielded. Install insulation/shield to prevent burns. 46 CFR 144.830.

The vessel struck an object while transiting, creating a 2' crack midship on the port side. The vessel is required to submit a repair proposal to the OCMI prior to commencing repairs and make the vessel available to attending Coast Guard marine inspectors. All repairs are to be to the satisfaction of the attending Coast Guard marine inspector.
