The arrangements for the storage, distribution and utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons on board. PSCO's observed abnormal quanitity of oil in the main engine galleries and on the port forward corner. 74SOLAS(2001) 11-2/15.3

Resolved 2 April 2026
Resolution: Vessel cleaned oil and changed drain line seals on the main engine to prevent future leaks.

Electrical installations shall be such that:... the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO's observed multiple extension cords that were in permanent use for various lights, fans, and a pump throughout the engine room, purifier space, workshop, and CO2 room. 74SOLAS(2004) II-1/40.1.3

Resolution: Vessel removed all extension cords that were being utilized in an incorrect manner.

...Any other machinery space shall be adequetaly ventilated appropriate for the purpose of that machinery space. PSCO's observed a portable ventilation trunk over a parts cleaning table in the purifier room. The other end of the trunk was attached to a piece of wood which was blocking the main purifier space exhaust ventilation. 74SOLAS(2004) II-1/35

Resolved 2 April 2026
Resolution: Vessel removed the portable ventilation trunk and wood to restore normal ventilation in the purifier room.

An immersion suit of an appropriate size complying with requirements of section 2.3 of the code shall be provided for every person on board the ship. PSCO's observed immersion suits were too large for some crew members on board. 74SOLAS(20) III/32.3.2

Resolution: Received a satisfactory class report from [name withheld] stating that immersion suits of appropriate size for the crew has been place onboard the vessel.

1730 - Any failure of the oil filtering equipment shall be recorded in the Oil Record Book Part 1. On 21NOV2018 the entry in the Oil [name withheld] indicated that the oily water sperarator ran for 2 hours and 25 mins processing 8.7 cubic meters of oily water which is a through put of 3.6 m/hr. This eceeds the throughput as listed on the IOPP of 2.5 m/hr. 16a - MARPOL 73/78 Annex1/Reg 17.5

0725 - Maintanance testing and inspections shall be carried out based on the gudelines developed by the organization and in a manner havin due regard to ensuring the reliablity of fire fighting systems and appliances. Two (02) CO2 flexible hoses were cracking in the rubber outer layer. Multiple hoses were beginning to kink and many bottles were showing signs of rotating in their mounts. 74SOLAS(2014) II-2/14.2.2.1
