Each transfer pipe system, including each metallic hose, must not leak under a static liquid pressure at least 1.5 times the MAWP. The last time the vessel tested to 1.5 times MAWP of their transfer hoses was 16OCT2014. The time since last test date is in excess of the testing frequency required. 33CFR156.170 15c

Resolution: Vessel sent in proper documentation and pictures showing the the cargo lines had been tested at 150% MAWP. No issues noted, no pollution noted.

Resolution: Coast Guard is giving an exception to allow the vessel’s departure from this port having only completed the 100% MAWP pressure test. A new deficiency will be issued that will replace deficiency number 01. Vessel is permitted to coduct cargo ops.

1099-Pressure sensors may be fitted in each tank protected by the arrangement required in paragraph 6.1.2, with a monitoring system in the ships cargo control room. Such monitoring equipman shall also provide an alarm facility which is activated by detection of over pressure or under pressure conditions within the tank. Currently the #4S pressure sensor was indicating a pressure of 6 mBAR. When checked locally by the crew using portable equipment sensor showed 700 mBAR. This proves the system is not operating properly. Vessel must provide USCG with a corrective action plan on how vessel will monitor tank pressure. 74SOLAS(2004) II-2/11.6.3 50abc

Resolution: Attending DNV GL Surveyor has verified repairs to vessels cargo oil tank remote monitoring system and all tanks No. 1 P/S through No. 6 P/S and sighted all as fully functional. Repair considered permanent and complete at this time.
