The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship and its equipment are fit to proceed to see without danger to the ship or persons on board. PSCO observed the front cover of the electrical panel for VDM/CCM removed and a fabricated ventilation system installed to prevent the system from overheating. Additionally, observed discrepancy between the temperature gauges for exhaust gas located on the main engine,and at the remote display located in the engine control room. 74SOLAS(20)1/11(a)

Resolution: CCS provided report No. HT23SS00042 attesting to rectification. Specifically, ventilation has been removed, cover reinstalled, and temperature guages cleaned ensuring operation.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO identified exposed 220v wire inside deteriorated reefer container connection box. Additionally, multiple 220v connections on the starboard side cargo holds were found uncapped and in boxes with deteriorated seals and cable penetrations. 74SOLAS(04)II-I/45.1.3

At appr. 0820 local, PSCO received notification that the vessel experienced an issue with its air pressure control system when inbound to its anchorage.

PSCO's found a pinhole leak in the sea valve cooling pipe for anchor hydraulic pumping system. -SOLAS(04)II-1/26.1

Vessel suffered a fire in port side cargo hold #3. Vessel crew closed ventilation to cargo hold#3 and discharged 59 bottles from the ships fixed CO2 system. the extent of the damage caused by the fire is unknown. 74SOLAS(14)I/11

Resolution: Reviewed CCS class report stating no damage was found in the cargo hold and C02 bottles have been recharged/replaced. Attached class report to vessel documents.

Whenever there is a hazardous condition either on board a vessel or caused by a vessel or its operation the owner, agent, master, operator, or person in charge must immediately notify the nearest Coast Guard Sector office or Group office and in addition submit any report required by 46CFR4.05-10. Vessel suffered a fire in cargo hold#3 on 23MAY18 at 0012. No notification was made until 0800 failing to immediately report or caused to be reported. 33CFR160.216
