Whenever there is a hazardous condition either on board a vessel or caused by a vessel or its operation, the owner, agent, master, operator, or person in charge must immediately notify the nearest Coast Guard Sector Office or Group Office, and in addition submit any report required by 46 CFR 4.05-10. Vessel did not report to the nearest Coast Guard unit when it was found that there was a crewmember who tested positive for COVID-19. The vessel failed to notify until two days later. 33CFR160.216(a)

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. PSCO observed a non class approved epoxy repair to the main diesel engine cooling water line. Conduct repairs to the satisfaction of certificate issuing authority prior to 1200 25 June 2016.

A ship engaged in the carriage of dangerous goods in any cargo spaces shall be provided with a fixed carbon dioxide or inert gas fire extinguishing system complying with the FSS code. PSCO observed significatn wastage to fixed fire suprresion lines to cargo holds. Request ceritificate issuing authority to contest to continuing acceptance.

The crew has been burning trash in the incinerator on a regular basic. However, from May 06 until now the [name withheld] has not been recording the date, time and location in the garbage log. There have been approximately 10, recorded uses in the oil record book documenting the burning of oily rags.

The crew has not been separating the plastic tops from the juice containers or glass jars. They are not following the garbage plan when disposing of garbage in the appropriate color coded containers. In the engine room they are using blue containers for oily rags, which are required to be black.

The smoke detector above the feed water tank was covered with a plastic bag. The crew covered it because it would go off when steam rose from the tank. We spoke to the Master and Chief Engineer and [name withheld] about the safety issues regarding the covering of this detector. Recommended the Master discuss with company about relocating the detector.

Some type of non slip surface needs to be installed in the steering flat. There is a small railing however, it does not lead directly to the controls for the steering unit. Plus the control units are inside the containment area and there isn't anything for the crew to stand on if there was a hydraulic leak.
