Whenever there is a hazardous condition either on board a vessel or caused by a vessel or its operation, the owner, agent, master, operator, or person in charge must immediately notify the nearest Coast Guard Sector Office or Group Office, and in addition submit any report required by 46 CFR 4.05-10. Vessel did not report to the nearest Coast Guard unit when it was found that there was a crewmember who tested positive for COVID-19. The vessel failed to notify until two days later. 33CFR160.216(a)

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. PSCO observed a non class approved epoxy repair to the main diesel engine cooling water line. Conduct repairs to the satisfaction of certificate issuing authority prior to 1200 25 June 2016.

Resolution: Class report approved temporary repair but required permanent repair to be completed by 24AUG2016. Current repair was completed with a non spec pipe. Permanent repair to be made with a coated and original thickness pipe.

A ship engaged in the carriage of dangerous goods in any cargo spaces shall be provided with a fixed carbon dioxide or inert gas fire extinguishing system complying with the FSS code. PSCO observed significatn wastage to fixed fire suprresion lines to cargo holds. Request ceritificate issuing authority to contest to continuing acceptance.
