07123- Operation of Fire Protection Systems- Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500 liters and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. PSCO observed a blocking device in the remote quick closing valves on the LSMGO service tank. 01 valve was the supply to the ship service diesel generators. Ships crew stated the devices were placed in the valves to test quick closing valves on August 27, 2018 and were never removed. SOLAS 74 (04 CONS) II-2/4.2.2.3.4 30a

Resolution: 18OCT18: Received class report from NKK stating all outstanding deficiencies have been rectified. The class report stated: quick closing valve bolt was removed and operationally tested satisfactory. The vessel was released from IMO Detention.

07126- Oil Accumulation in Engine Room- Means shall be provided to control leaks of flammable liquids. PSCOs observed missing handles for manual fuel valves for #1, #3, #4 and #5 at the main engine. Crew immediately reattached handles and opend all fuel valves to each cylinder which resulted in excessive leaks on main engine fuel injectors on the #1, 3, 4, and 5 cylinders.
