Fuel lines must be protected from mechanical injury and effectively secured against excessive movement and vibration. Observed rigid fuel line on port side MDE chafing against fuel shutoff. Install chafing gear.

Fill pipes and sounding pipes must be so arranged that overflow of liquid or vapor cannot escape to the inside of the vessel. Observed leak in fuel fill line. Replace with approved hose.

Flexible hose used in fuel oil systems shall have hose clamps installed. Observed fuel hose in engine space missing clamps at fitting. Install clamps at both ends of the hose.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from machinery failure. Serpentine belt causing high-pitched squeal during startup due to excessive wear. Replace belt.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Observed emergency instructions not present aboard. Vsl will create emergency instruction document.

Each lifejacket must be approved under appropriate approval series subparts. Observed retro reflective tape on child life preservers incorrect type. Replace with SOLAS tape.

A vessel must carry one floating water light, unless it is limited to daytime operation, in which case no floating water light is required. MI observed water light not operational. 46 CFR 180.72

A vessel of not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: The cognizant OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. MI Observed master unable to recover simulated helpless person in the water during man overboard drills. Vessel’s free board is in excess of two feet. Required to submit plans for approved rescue platform as outlined in MPS-WI-SECSP(spi)-14. Required to preform satisfactory drills in presence of USCG MI prior to carrying passengers. 46 CFR 180.210(b)(2); 701c

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil. MI observed oil leak on Main Diesel Engine port side, 1 drop per 5 secs. Required to resolve leak, remove oil in bilge, and submit plans for review that address mitigating actions to prevent oil discharge overboard through bilge system. 46 CFR 176.830(b); 701c

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Letters and numbers must be at least 50 millimeters (2 inches) high. MI observed markings inconsistent with quantity of life jackets stowed. Required to relabel IAW 46 CFR. 46 CFR 185.604(f); 701c

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. MI observed adult and child life jackets not separated within storage under seating. Required to segregate. 46 CFR 180.78(a)(5); 701c

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI Observed side nav lights not operations. Restricted to fair weather, day light operations with min visibility of 3 nm. 46 CFR 183.420; 701c

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall have the vessel ready for inspections of the hull structure and its appurtenances, including all interior spaces to ensure that they are adequately ventilated and drained. MI observed more than 3 inches of oily water in bilge. Required to remove and dry all voids prior to vessel being laced in water to transit to alternative Drydock facility to conduct repairs identified. 46 CFR 176.802(7); 60c

During the examination all accessible parts of the vessel's underwater body and all through hull fittings shall be made available for examination. MI observed bilge overboard valve not operational IAW 46 CFR 182.25-5(c). Vessel may transit to alternative Drydock facility to conduct repairs identified. Required to repair prior to carriage of passengers. 46 CFR 176.610(a); 701c

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel was due for drydock examination on 31DEC2024 and has yet to receive a drydock exam. 46CFR176.600(c)(1); 701c

Vessel must undergo an annual inspection as specified in paragraph (b)(1) of this section. Vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessels anniversary date was 11MAR, and vessel is past 3 months from said date. Vessel shall not operate with passengers until an annual inspection has been completed. 46CFR176.500(b); 701c

46 CFR 182.100 All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI observed that main diesel engine fuel injectors fuel return line had failed and was leaking. Vessel shall enact repairs to the satisfaction of attending USCG Marine Inspector.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel was unable to start engine and had to be towed to pier. Determine source of failure and repair to the satisfaction of the MI.

Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. Where flexibility is necessary, a section of flexible metallic hose may be used. Nonmetallic hose may be used for wet exhaust systems provided it is especially adapted to resist the action of oil, acid, and heat, has a wall thickness sufficient to prevent collapsing or panting, and is double clamped where practicable. MI observed close nipple on main exhaust weld seam was corroded and appeared to leak saltwater, non-metallic hose has heavy saltwater scale at the end of hose clamps, there was exhaust smoke leaking in the same area as well. 46 CFR 182.430(b)(e)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. MI observed approximately 15 gallons of oily water mixture in the bilge including diesel fuel. Owner removed contents immediately and wiped down the forward compartment bilge area. Deficiency corrected on the spot. 46 CFR 176.830(b)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. MI observed oil throughout engine room on the engine and in the bilges. Vessel in previous inspections has been observed in the same state. Owner to provide a plan to troubleshoot and find locations where oil is coming from and provide repair proposal to CG. 46 CFR 176.830(a)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. After a deficiency was issued during the annual inspection completed 17JAN23, MI approved a new master only plan that required the vessel rep to install a set of second throttles near the emergency steering station. The throttles have still not been installed to this date and there is not estimated time of repair. Vessel is prohibited from conducting master only operations until this is rectified and approved by the CG. 46 CFR 176.840

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI observed bow combination red/green light was not functioning properly, producing excessive heat and serving as a hazard to safety and navigation. 46 CFR 183.420 705c (Daylight Operations Only)

Each battery used in an item of lifesaving equipment must be replaced during the annual inspection if the expiration date of the battery has passed. MI observed floating waterlight battery for ring life buoy was expired. 46 CFR 185.726(d) 705c (Daylight Operations Only)

The [magnetic] compass must be illuminated. MI observed that magnetic compass was not illuminated. 46 CFR 184.402(c) 705c (Daylight Operations Only)

On a vessel of at least 26 ft in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. MI observed that visual alarm was functional, but no audible alarm was heard while testing bilge high level alarms. 46 CFR 182.530(a)

All vessels shall be provided with bilge pumps in accordance with Table 182.25-10(a). Vessel is required to have 1 fixed power pump and 1 portable hand pump as per the table. MI observed no portable hand pump onboard. 46 CFR 182.25-10(a)

It is intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. MI observed mounting bracket for the solenoid for the main engine shutdown was not suitably attached to the engine. 46 CFR 182.01-5

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Owner is in the process of changing consortiums from the Maritime Consortium to APCA. Provide proof of enrollment for all crewmembers in APCA. 46 CFR 16.230(a)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. MI observed the last drill/training entry in the log was from March of 2022. Vessel rep stated that drills/training occur approximately every 3 months, but have not been logged. 46 CFR 185.520(f)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: charts, US Coast Pilot [name withheld] List, Tide Tables, and Current Tables. MI observed all of the listed references were out-of-date. 46 CFR 184.420(a)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. MI observed that vessel's Master's Only Operation Plan dated 04JUN2015 did not reflect updated operating information required by the local unit. In addition, the plan was not posted onboard as required per the COI. Resubmit updated plan to OCMI reflecting current operations for review/approval. 46 CFR 176.840

Vessel was due for drydock examination on 31DEC22 and has yet to receive a drydock exam. 46 CFR 176.600(c)(1)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. MI observed approximately 30 gallons of diesel fuel in the bilge of the forward compartment. Significant accumulation of oil has been noted in this compartment in the past 3 inspections of the vessel, to include an annual inspection on 09JUN22, a COI inspection on 25MAR21, and a hull/ISE inspection on 23OCT20. Clean and properly dispose of fuel, determine source of leak, and provide repair plan. 46 CFR 176.830(b)

Propulsion machinery must be suitable in type and design for the propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The operator reported a loss of propulsion while maneuvering in Sparkman Channel, Tampa, FL. Provide the attending USCG Marine Inspector with a repair proposal and make repairs to the satisfaction of the OCMI. 46 CFR 182.200(a) and 46 CFR 176.700(a)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil. Excessive accumulation of oil, up to 2.5 inches at the deepest point, observed in fwd bilge. 46 CFR 176.830(b)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Retroreflective material on 01 child life jacket was deteriorated. Vessel limited to 03 children until corrected. 46 CFR 185.604(h)

A vessel on a lakes, bays, and sounds route must carry three hand red flare distress signals approved IAW 160.021 and three hand orange smoke distress signals approved IAW 160.021.Hand red flare distress signals may be substituted for hand orange smoke distress signals. Vessel had 04 red flares.

11118 - Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Retroreflective material on life jackets was no longer reflective. Vessel limited to 20 passengers until corrected. 46 CFR 185.604(h)

07123 - A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. During test of pre-engineered system, engine was able to restart without engaging override function. 46 CFR 181.420(a)(4)

07126 - At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Excessive accumulation of oil, up to 3.5 inches at the deepest point, observed in fwd bilge. 46 CFR 176.830(b)

09203 - Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Port aft light was exposed/uncovered. 46 CFR 183.410(a)

11117 - At least one ring life buoy must be fitted with a lifeline ... Each lifeline on a ring life buoy must have a breaking strength of at least 5 kilonewtons (1,124 pounds). Operator replaced frayed life line. 46 CFR 180.70(c)(5)

13199 - Flexible hose runs must be visible, easily accessible, protected from mechanical damage, and must not penetrate watertight decks or bulkheads. Operator replaced rubber protection of hose in the engine room to prevent future damage. 46 CFR 182.720 (e)(3)(ii)

46 CFR 176.600(c)(1) A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel did not conduct a scheduled dry dock due on September 30, 2019. Prior to carrying passengers, vessel must complete a dry dock exam in the presence of a Coast Guard Marine Inspector.

46 CFR 176.100 A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. The vessel's Certificate of Inspection expired on February 03 2020. In accordance with 46 CFR 176.404(b), you must submit your written application for renewal of a Certificate of Inspection to the OCMI at least 30 days prior to the expiration date of the Certificate of Inspection, as required in 176.105. Prior to carrying passengers, submit an Application for Inspection (CG3752) and complete a Reinspection of the Certificate of Inspection.

Electrical submersible bilge pumps must be installed with a positive shut off valve at the hull penetration. Install metallic valve.

Apply retro reflective material to life ring buoy as required.

Replace expired ammonia inhalants in first aid kit as required.

Provide audible and visual alarm for high level indicator and visual alarm for automatic bilge iaw 46CFR182.530(a)(b)

Provide PVC or manufacturers mount for fir extinguisher iaw 46CFR181.500.

Repair deck rail for stair well iaw 46CFR177.35-1.

Remove dead end wires/ organize wires at operating station iaw 46CFR183.01-15.
