Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces at least one of which shall give access to a stairway forming a vertical escape. PSCOs observed an escape door that was jammed in the 3rd deck midship conference room space, called the "The Meeting Space" had a partition that served as an escape route that was wedged shut.

FSD 10.24 was equipped with an alarm, however, the door moved at the same time the alarm was activated.

Where pipes, electric cables, etc. are carried through watertight subdivision bulkheads, arrangements shall be made to ensure the watertight integrity of the bulkhead. PSCO observed muultiple on-metallic pipes penetrating wateright bulkheads, above watertight doors 1-10 on the tween deck, two decks below the bulkhead deck. No approved plans were provided by the ship.
