PSCO observed the sensor for the gas sampling system OGS 30/17 not operating within established procedures/tolerances. Sensor could not be calibrated and read 100% LEL when tested with 60% LEL span gas. Sensor needed to be replaced. Similar issues existed when tested by USCG on 04 DEC 2019 (16 days prior). 06105 - Vapor detection equipment should be activated when the vapor concentration reaches the equivalent of 30% of the LEL in air or such other limit as may be approved by the administration in the light of particular cargo containment arrangements. The vessels cargo containment system is of the membrane design. The primary and secondary barrier spaces are foam insulated. The primary barrier space vapor detection alarm is set to alarm at 10 to 30% of methane by volume instead of 30% LEL. Provide attestation that the administration has evaluated the vapor detection installation and verified that it meets the requirements of IGC 13.6.11 74SOLAS 14 VII/3.1 IGC 93/13.6.11 50bc

All ships subject to the code shall be fitted with a stability instrument, capable of verifying compliance with intact damage and stability requirements, approved by the administration. Ships constructed before 1 July 2016 shall comply with this paragraph at the first scheduled renewal survey of the ship after 1 July 2016. Subject vessel experienced renewal survey on 30 April 2018 but has no approved stability instrument onboard. Certificate of Fitness dated 30 APR 2018 does not denote any loading conditions I.A.W. section 6.1,6.2, or 6.3 of COF.
