PSCO observed active fuel leaking from M/E cylinder #5 fuel pump and #2 MDG fuel line. Additionally, PSCO observed oil accumulation under the turbo on both #2 and #4 MDG’s

The machinery boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed active hydraulic leaks and accumulation under both steering gear pumps. Additionally, oil accumulation was found inside the remote control valve unit panel.
