Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels. No program available upon request. 46 CFR 16.230

A hatch exposed to the weather must be watertight. Vessel had multiple weather deck hatch/covers where the gasket material was worn down enough to allow down-flooding into spaces/voids. Owner/Operator must replace/repair all the gasket material throughout the vessel to ensure watertight integrity. 46 CFR 179.360(a)

Portable waterlight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters…Container for flares was of black color. 46 CFR 185.614

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: […] a bilge well. The stbd fwd bilge pump was not producing an alarm for high water levels or pumping water. 46 CFR 182.520

The owner, charter, master or managing operator shall instruct each crew member…at least once every three months, as to the duties that the crew member is expected to perform in an emergency…Crew training shall be logged or otherwise documented for review…Maintenance and training logs were unable to be provided at time of inspection. 46 CFR 185.420

A vessel not required to have a power driven fire pump…must have at least three 9.5 liter (2 ½ gallon) buckets, with an attached lanyard…placed so as to be easily available during an emergency. Vessel only had 02 fire buckets on board. 46 CFR 181.610

The light must have a floating waterlight approved under approval series 161.010 or other standard specified by the Commandant. The floating waterlight must be attached around the body of the life float…Lifefloat light battery was inoperable and light was not connected to anything. 46 CFR 180.175(f)(4)

Each…ring life buoy must be marked in clearly legible block capital letters with the vessel’s name. 01 ring life buoy was found with no name, and 01 ring life buoy was found with an abbreviated name instead of the vessel’s official name. 46 CFR 185.604(b)

A vessel must carry either a fire-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel had several first aid kits, two of which contained expired contents. Vessel must remove all expired contents from first aid kits. 46 CFR 184.710

Both life rings (02) do not have vessel name clearly labeled. Life rings need have vessel name clearly visible. 46CFR180.70

Radio Station Authorization has wrong Vessel Official Number listed. Certificate needs to be updated to reflect correct official number. 47CFR80.1000

Vessel log and Drill log were last written in back in 2019. Maintain current vessel logs that are up to date. 46CFR185.728

Starboard side (midships) void had excess water. Hatch needs to be made watertight. 46CFR179.330

Certificate of Inspection needs to be posted on board. If posting is impracticable, the certificate must be kept on board in a weathertight container readily available for use by the crew and display to passengers on request. 46CFR176.302

Remote fuel shutoff station marking is not clearly legible. Remote fuel shutoff station must be marked in clearly legible letters at least 1 inch high.46CFR182.435

46CFR67.7 / 701c - Any vessel of at least 5 net tons which engages in coastwise trade must have a certificate of documentation. No valid COD was onboard vessel at the time of inspection.

46CFR185.608 / 16c - Remote fuel shut-off stations must be marked in clearly legible letters of atleast 1" high indicating purpose of the valve and direction of operation. Fuel shut offs were not marked appropriately.

46CFR185.604 / 16c - Each life jacket must be marked in clearly legible block capital letters with the vessel's name. 2 lifejackets found without appropriate markings.

46CFR180.68 / 701c - A vessel on a lakes, bays, and sounds route must carry 3 red hand flares and 3 orange smoke distress signals. Vessel did not have any distress flares onboard.

46CFR179.360 / 701c - A hatch exposed to the weather must be watertight. Aft hatches on starboard and port side, adjacent to operating station have no installed gaskets and have had hatch hinges disassebled.

THE OFFICER IN CHARGE, MARINE INSPECTION (OCMI) OF SECTOR PUGET SOUND HAS RECEIVED CREDIBLE REPORTS OF THE SUBJECT VESSEL OPERATING AT SPEEDS WELL IN EXCESS OF 30 KNOTS. THIS VESSEL HAS NOT BEEN APPROVED FOR HIGH SPEED CRAFT SERVICE AND IS NOT AUTHORIZED TO EXCEED 30 KNOTS. ADDITIONALLY, THE VESSEL'S STRUCTURE APPROVALS ARE BASED ON A 35 KNOT SPEED LIMITATION. OPERATING AT SPEEDS GREATER THAN 30 KNOTS EXCEEDS THE APPROVALS GIVEN BY THE COAST GUARD FOR THIS VESSEL. YOU MAY NOT OPERATE BLACKFISH (ON 1175354) AT SPEEDS GREATER THAN 30 KNOTS AND MUST MAKE THE VESSEL AVAILABLE AND COMPLETE AN INTERNAL STRUCTURAL EXAM WITHIN 60 DAYS TO THE SATISFACTION OF THE MARINE INSPECTOR. At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct test and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following : Test of machinery alarms including bilge high level alarm. The bilge high level alarms were inoperative.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. The float switch for the bilge pumps were inoperative.

The owner, charter, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by 185.510. Vessels master was unfamiliar with bilge pump system and the associated breaker for the system.
