Vessel must undergo an annual inspection as specified in paragraph (b)(1) of this section. Vessel must undergo an annual inspection within 3 months before or after the each anniversary date. Vessels anniversary date was 04DEC2025, and vessel is past 3 months from said date. Vessel shall not operate with passengers until an annual inspection has been complete. 46 CFR 176.500

Each ring life buoy must be marked with Type I or II retro-reflective material approved under subpart 164.018 of this chapter. MI observed peeling and deteriorated retro-reflective tape on ring life buoy. Recommend replacing tape with approved Type I or II material per 164.018 standards. 46CFR131.875

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephone Certificate. MI observed that the FCC radiotelephone certificate onboard expired. 47CFR80.903

All marine employers must submit drug testing program data required by 49 CFR 40.26. Observed that the Management Information System (MIS) drug testing report for previous year was missing from the vessel. Submit MIS report for 2025 to marine inspector. 46CFR16.500

Each floating water light must be approved IAW 161.010 or 160.110. The light onboard doesn't work and is not of the approved type. Vessel is restricted to daylight operations until the proper floating water light is properly rigged and onboard. The vessel may operate under this restriction for 14 days. 46 CFR 180.70

A vessel must comply with the applicable requirements for any radio installations as set forth in 47 CFR part 80. The radio was not operational. Vessel is limited to operations within 1000' feet from shore until the VHF radio is operational. 46 CFR 184.502

A durable placard must be posted next to all radio installations with emergency broadcast instructions and information specific to the vessel. No emergency broadcast placard was posted. 46 CFR 184.510 outlines what the instructions must include. Vessel has 14 days to comply. 46 CFR 184.506 46 CFR 184.510

At each inspection the vessel shall be prepared to conduct tests of machinery to include machinery cooling systems. The MI observed the flushing hoses for both outboards were leaking water when the engines are running. Repair to stop water leaks. 46 CFR 176.804

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel was due for drydock examination on 31MAR25 and has yet to receive a drydock exam. Vessel shall not operate with passengers until an drydock exam has been completed. 46 CFR 176.600 (c)(1)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Marine Inspector identified burnt wiring coming from bottom 12vdc battery going to port side fuel tank grounding tab. Have the wire replaced and attested by certification of certified marine mechanic/electrician. 46 CFR 176.830(a)

The owner shall submit for approval to the cognizant OCMI plans for electrical installations. (4) Electrical installation including, but not limited to: (i) Elementary one-line diagram of the power system; (ii) Cable lists; (iii) Bills of materials; (iv) Type and size of generators and prime movers; (v) Type and size of generator cables, bus-tie cables, feeders, and branch circuit cables; (vi) Power, lighting, and interior communication panelboards with number of circuits and rating of energy consuming devices; (vii) Type of capacity of storage batteries; (viii) Rating of circuit breakers and switches, interrupting capacity of circuit breakers, and rating and setting of overcurrent devices; and (ix) Electrical plant load analysis. Marine Inspector identified additional components installed including battery charger, power inverter, refrigerator wiring, and exterior going around vessel. Provide the Coast Guard with approved electrical diagrams for the vessel. ***Owner is required to provide approved electrical diagrams in (07) seven days.*** 46 CFR 177.202(b)(4)

Except on a vessel limited to daytime operations, the compass must be illuminated. Marine Inspector identified compass light was not visible due to not being connected. ***Vessel is reduced to daytime operations only.*** 46 CFR 184.402(c)

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. Vessel had incorrect stability letter posted. Post new stability letter including all pages. ***Correct within (07) seven days.*** 46 CFR 176.306

EAP training program: An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Marine Inspector conducted DAPI audit finding that the Supervisor had not completed formal training consisting of 60 minutes of training to qualify in the Supervisory position. ***Correct within (07) seven days.*** 46 CFR 16.401(b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. (b) Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Marine Inspector identified all charts, pubs, coast pilot, tides and current tables were all outdated. ***Correct within (07) seven days.*** 46 CFR 184.420(a)(b)

Lakes, bays, and sounds, and rivers routes. A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant; and (2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Marine Inspector identified (04) hand held red flares had expired. Owner purchased new flares to meet regulation. 46 CFR 180.68(b)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Marine Inspector identified ring buoys lanyard was dry rotted. Owner replaced immediately. 46 CFR 180.70(d)(2)

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel was due for renewal COI inspection on 13NOV23 and has yet to receive a renewal COI inspection. Vessel shall not operate with passengers until new COI has been issued. 46 CFR 176.100

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel's anniversary date is November 13th. Vessel did not complete annual inspection within 3 months of the anniversary date. 46 CFR 176.500(b)(1) 701c

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years to receive a Communications Act Safety Radiotelephony Certificate. Vessel did not have an Safety Radiotelephony Certificate. 46 CFR 184.502 47 CFR 80.903

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a marine radio operator permit. The operator did not have a marine radio operator permit. 46 CFR 184.502 47 CFR 80.159(e)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked "First-Aid Kit." Vessel's first aid kit was not approved under approval series 160.041 nor was it a kit with equivalent contents and instructions. 46CFR184.710

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. A battery cable was missing conduit at the connection point to the battery. 46 CFR 183.200 (b)

Dead end wires were found coming from the VHF radio under the operating station. 46 CFR 183.200 (b)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operation station and the other at the access to the propulsion machinery space. Vessel was missing one of its required flashlights. 46 CFR 183.430

A vessel must carry adequate and up-to-date U.S. Coast Pilot and Coast Guard Light List. Extracts from these publications for the areas to be transited may be provided instead of the complete publication. Vessel did not have a Light List onboard. 46 CFR 184.420 (a)(3)

Replace Gasoline Fuel Lines with approved USCG Type A1, A2, B1, or B2 Flexible Hose. IAW:46 CFR 182.720 (e)

Certificate of Documentation on board vessel was expired. Provide C.O.D. that is current for vessel. IAW: 46 CFR 67.313

Lifejackets stowed in a way they were unable to be readily accessed. Prove proper stowage to the satisfaction of the marine inspector IAW 46 CFR 180.78
