Oil filtering equipment referred to in paragraph 2 of this regulatoin shall comply with paragraph 6 of this regulation. It was observed on 16SEP19 the vessel discharged 41.62 cubic meters of bilge water through the OWS in 8 hours (0900-1700) exceeding the max through put (5 cubic meters/hour). While examining the OWS the chief engineer could not show the recirculating function. After priming the system and opening the recirculation valve the system over pressurized, breaking multiple gaskets and a pressure gauge on the system. System was not tested satisfactorily for the PSCO

Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and selfclosing. The A/C room on the upper deck and the galley A/C room on A deck, which by definition in SOLAS are machinery spaces of Category A. Neither door is fitted on an "A-0" and "A-60" door in sterring gear room. Finally another A-0 door to steering gear was unable to close properly via the self closing device.

15 - Safety Management Systems (ISM) › N/A - No Subsystem › Masters responsibility and authority

The company should ensure that the safety management system operating on board the ship contains clear statement emphasizing the master's authority. The company should establish in the safety management system that the master has the overriding authority and the responsibliity to make decisions with respect to safety and pollution prevention and to request the company's assistance as may be necessary. While lowering the port side lifeboat the crew did not install the required fall prevention device. Additionally the fire fighters left their required radios in the fire locker and did not bring them to the drill. Additionally, it was observed the vessel procured new washing machines last summer. The new washers are 110V and the ship supplies 220V. The crew spliced multiple lines to wire the machines without approval from class. During the engine room exam the PSCO observed a makeshift catch (bucket) underneath the stern tube filled with oil. The cief engineer could not explain why it was there and the company has no established procedures for logging soundings of the sterntube tank. During the test of local steering the crew was unaware of the presence of the rudder angle indicator.Rather, a crew member climbed on top of the steering gear to observe the rudder indicator on top of the rudder stock. Recommend an ISM audit.
