A-CLASS RATED FIRE DOORS IN THE GALLEY WERE FOUND TO HAVE HOLD-BACK HOOKS NOT SUBJECT TO CENTRAL CONTROL STATION RELEASE.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel reported a loss of propulsion due to low oil level in cam shaft reservoir. Vessel is required to provide a satisfactory class report prior to movement from berth.

Main and auxiliary machinery essential for the propulsion and safety of the ship shall be provided with effective means for its operation and control. Effect repairs to restore propulsion plant to operate as designed prove proper operation of plant. SOLAS 74 II-1/31.1

Received notification from ABS surveyor [name withheld] that the OWS aft filter casing had a minor leakage in the vertical weld. Oil filtering equipment shall be of a designed approved by the administration and shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 ppm. VSL used hard epoxy compound as a temporary repair. This is to the satisfaction of ABS who witnessed testing. ABS issued a short term IOPP for 2 months to make repairs at next port of call. USCG requires a pre-inspection and post-inspection of temporary repairs on OWS each time operated. Make permanent repairs to to the approved design of the OWS at next port of call to the satisfaction of attending USCG inspector or ABS Surveyor. MARPOL I/14.6

Periodic Safety Procedures (PSTP) shall be approved by the Marine Safety center or the Authorized classification Society (ABS) and be retained onboard the vessel. The form of approval shall be a letter of approval recommendation from MSC or a stamped copy of the PSTPs in accordance with 46 CFR 61.40-1. The Vessel cannot provide the approval letter or approved PSTPs for Coast Guard review during inspection. The COI manning shall be modified, with the addition of (1) Oiler to accommodate the watch station requirements. The PSTPs shall be completed in its entirety by the crew. Once the PSTPs have been completed and updates/revisions reviewed and approved by MSC or ABS, the crew must notify the local OCMI to request a re-inspection for authorization of PUMs.

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate means of starting, instead relying on paralleling of both battery banks. Replace/repair to original manufacturer condition. SOLAS 74 a (1997) III/19.2

Incinerator doors are missing fire gasket, repair as original. SOLAS 74a (2014) I/11

The fire resistance of doors shall, as far as practicable shall be equivalent to that of the division in O6 engine room emergency escape A-60 self closing door preventing closure. SOLAS 74 a (1997) II/47

Fire damper covers were not properly aligned. Deficiency was corrected on the spot.

Vessel has failed to report via LRIT per 33 CFR 169.230.

During drill, crews unfamiliar with SCBA and how to fight fire.

Electrical connections for the follow up mode of either pump 1 or pump 2 were not operating correctly.

All life-saving appliances shall be in working order and ready for immediate use. At the fall release mechanism the aft fall release cable has slipped from the cramped connection on both the port and starboard lifeboats. SOLAS (2014) III/20.2 Due: Next U.S. Port but not later than 20 Jan 2017.

Life-saving appliances shall be approved by the Administration. The lifejacket and survival suits were found not to be approved under 46 CFR 161.112. A total of thirty-nine (39) lifejackets and thirty-seven (37) survival suits. 46 CFR 199.70, SOLAS III/4.1.

The oil content meter will be calibrated and zeroed as per the manufactures instructions. Provide valid calibration certificate for onboard oil content meter. Oily Water Separator shall remain secured and out of service until new calibration is provided and tested in the presence of an ABS Surveyor or Marine Inspector. MARPOL Annex I Reg. 14.7, MEPC 60(33) 2.2.5.

Effect repairs to the starboard lifeboat davit to the satisfaction of the attending Coast Guard Marine Inspector by 15 Dec 16

Vessel could not provide gap analysis as required for MSP Re-Flag. Provide vessels Gap Analysis or equivalent approval letter in accordance with MSC NVIC 01-13 Change 1.

Vessel does not have USCG approved hyrodstatic releases for life-rafts or EPIRB. Provide USCG/SOLAS approved hydrostatic releases for life-rafts and EPIRB showing USCG par 160 approval number

Rewire overhead lights in steering gear flat.

Renew/Replace 2nd chamber on OWS prior to use.

Amend/correct operating instructions for CO2 system in manual/local control to include zone master control valve bleeder operation.

Provide six lifejacked for the aft liferaft required by SOLAS 74 III.26.1.4. To be stored in a location close to the liferafte and marked on the lifesaving plan. SOLAS III.7.2.1.2 (1997)

Operator failed to notify the Coast Guard of dry-docking. LOW to be issued.

Failed to notify the USCG of hull repair to the vessel. LOW to be issued.

Repair liquid CO2 level indicator.

Replace plastic zip tie wiring support in circuit board room and paper product stores rooms with steel ties (minimum 5/8", every 2 feet) within 30 days.

Repair A-60 Fire boundary above #1 service generator within 30 days.

Relocate fuel return piping for emergency generator from structural fire protection insulation (A-60) bulkhead to fire locker. Submit photos of repair to USCG by email.

Engineers could not demonstrate operation of overspeeds for main generators citing problem with governors. Provide report of servicing and satisfactory testing (at the generator vice relay) within 2 months.

Add survival suit to storage locker at FWD mooring station prior to sailing. (Update lifesaving plan).

Repair fuel supply piping leaks in emergency generator room within 14 days.

Multiple portable fire extinguishers noted as unserviceable. Manufacturer's instructions not onboard in accordance with IMO Fire Safety Systems (FSS) Code, and IMO resolution A.602(15). Provide manufacturer's instructions for periodic servicing of portable fire extinguishers prior to sailing, email copy to USCG. Provide for immediate checking and servicing of all portable fire extinguishers.

Provide report of age and testing of foam concentrates for fixed local application system for fuel oil purifiers, and spare quantity maintained onboard in accordance with SOLAS FSS & MSC Circ. 850. Provide test report, or report of replacement to USCG.

Maintenance of onboard fire extinguishing appliances & systems is not in accordance with SOLAS required maintenance plan found onboard. Provide ISM report of non-conformity & corrective action to address lack of maintenance records onboard, condition of portables noted by attending inspectors, and future actions to improve maintenance plan.

Provide letter from ABS attesting to validity of type approval on OWS equipment as modified from onboard manufacturer's instructions (wiring to oil level probe in second chamber removed).

Mark all lifejackets and immersion suits with the name of the vessel prior to departure.

Fixed CO2 system does not meet US domestic requirements. Vessel must submit proposal top MSC within 90 days to address the requirments of MSC ltr E1-0505803.

Fixed CO2 system does not meet US domestic requirements. Vessel must meet the requirments of MSC ltr E1-0505803 by next scheduled dry dock.

Vessel must have all class documents onboard prior to operation.

Vessel must complete ISM and ISPS/MTSA auditswithin 120 days and provide results to OCMI.

Vessel must complete all required crew familiarization training prior to operation.
