Resolution: Received, reviewed and accepted class report from DNV-GL stating the deficiency was rectified. Rescue boat davit brake was cleaned, adjusted, and tested satisfactorily.

0735 - Personal Equipment - fire fighting - Axe with a handle provided with high-voltage insulation. PSCO observed that each firefighter did not have a fire axe included in their ensemble. The vessel's safety manual states that a short handled axe shall be provided and that long handled axes may also be provided. 74 SOLAS (04)II-2/10.1 FSS Code Chapt 3.2.1.1.5 16a

Resolution: Received, reviewed and accepted class report from DNV-GL stating the deficiency was rectified. DNV GL held a training session with the crew and the Captain ordered the proper fire axes.
