A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel is overdue for dry dock exam as of 31Jan2021

Unless otherwise specified, vessel owners must pay the fees required by this subpart before inspection or examination services are provided in accordance with 46 CFR 2.10-20(a). Vessel inspection user fees are not paid for inspection year 2022. Pay all user fees and provide documentary proof that payment was submitted before any further inspection or examination services are provided to the satisfaction of an attending Coast Guard Marine Inspector . 46 CFR 2.10-20(a)

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. The Certificate of Documentation currently being used by this vessel is expired and is also documented in the name of the previous owner. Apply for a new Certificate of Documentation under the current owner to the satisfaction of an attending Coast Guard Marine Inspector prior to July 08, 2022. 46 CFR 67.313 46 CFR 67.321

Make repairs and properly install emergency fuel shut offs in way of both fuel tanks as discussed to ensure flow of fuel is completely secured in the event of an emergency. Demonstrate proper operation of the fuel shutoffs to the satisfaction of an attending Coast Guard Marine Inspector. 46 CFR 182.455(b(4)

Each vessel must have a Communications Act, Safety Radiotelephony Certificate as a result of a survey of all communications equipment on board in accordance with 47 CFR 80.59. The vessel either could not produce a Safety Radio Telephony Certificate or the Certificate onboard was invalid because ownership of the vessel changed. Submit to a Communications Act survey and obtain a new Communications Act, Safety Radio Telephony Certificate to the satisfaction of an attending Coast Guard Marine Inspector prior to July 8, 2022.

No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer as required by 46 CFR 16.210. During a review of the Marine Employers drug testing records on board, there were several persons identified that have been serving in safety sensitive positions on board without submitting to a pre-employment chemical test. Provide documentation that all individuals that have been identified have submitted to and received a negative test result for the pre-employment drug test and that each individual is enrolled in a random chemical testing program with his current employer to the satisfaction of an attending Coast Guard Marine Inspector prior to July 8, 2022.

The Coast Guard shall evaluate emergency drills to ensure that each crew member is familiar with his or her duties in case of emergencies including response to fires, man overboard and abandon ship in accordance with 46 CFR 185.520 and 46 CFR 185.524. Because of material deficiencies on two of the company’s vessels, the Coast Guard was unable to witness emergency drills during this annual inspection. Owner must make crew available to verify underway operation of equipment and witness emergency drills in the presence of attending Coast Guard Marine Inspectors prior to July 8, 2022.

Unless otherwise specified in description or Action Code, The vessel will be restricted from carrying passengers after July 8th, 2022 if the deficienies issued on June 8th 2022 have not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 46CFR176.107(c)

Submersible electric bilge pump is permanently mounted. Vessel electric bilge pump is not permanently mounted. 46CFR182.530(e)(3)

Vessels bilge pump had twist on wire nuts that were not being correctly used per 46CFR183.340(i). Replace with pressure type connectors that meet UL 486(A) (IE. Not heat shrink connections) per 46CFR183.340(b).

Submersible electric bilge pump is listed by an independent laboratory as meeting the requirements in UL 1113. Vessel replaced exisiting bilge pump with a non UL approved type. 46CFR182.530(e)(1)

Owner must return the vessel to the approved plan installation or submit for approval of electrical installation plans utilizing MTN.01-03.CH-2 and PRG.E2-23.2021.05.26.Electrical Plans (Attached) to the Marine Safety Center (MSC). The vessel will be restricted from carrying passengers after 15JUL2022 if the plans are not submitted to MSC by that date. Upon approval of plans, the local Coast Guard inspectors will verify the workmanship and the installation matches what is on the plan to the satisfaction of the attending Coast Guard Marine Inspector. Provide a suitable first aid kit that is either approved under CG Approval 46 CFR 160.041 or ensure the first aid kit has all the required items as listed in the approval. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Provide proof of Fire fighting drills and training as required by 46CFR185.524. Firefighting drills and training shall be logged or documented with the date of the drill and training and general description of drill scenario and trading topics. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Provide proof of Man overboard and Abandon ship drills and training as required by 46CFR185.520. Abandon ship and man overboard drills and training shall be logged or documented with the date of the drill and training and general description of drill scenario and trading topics. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Provide proof crew training at least every 3 months as to the duties the crew member is expected to preform as required by 46CFR185.420. Crew training shall be logged or documented with the date and general description of the training topics. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Provide a Communications Act, Safety Radiotelephony Certificate in accordance with 47 CFR 80.411 to the satisfaction of an attending Coast Guard Marine Inspector. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Properly mark the storage location and amount of Adult and children's life jackets ensuring that they are separated so that children are not mistaken for adult life jackets in accordance with 46 CFR 185.604 and 46CFR180.78. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

The fuel shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Fuel shutoff valve for both tanks were not working, Corrected on the spot.

Engine room wire supports are not properly supported and does not meet minimum standard as described in 46 CFR 183.340(b)(4). Wiring must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight; A hatch on a vessel that operates only on protected waters. Ensure forward and port side midship hatch are weathertight. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Employers must maintain records of chemical tests as provided in 49 CFR40.333 and must make these records available to Coast Guard officials upon request. Provide records of negative and cancelled drug test results and alcohol test results with a concentration of less than 0.02 for the last year. Additionally provide records of alcohol test results indicating an alcohol concentration of 0.02 or greater and Records of verified positive drug test results for the last five years. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date; Light List. Light list is out of date. The vessel will be restricted from carrying passengers after July 9th 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

By March 15 of the year following the collection of the data in paragraph (a) of this section, marine employers must submit the data on the form titled U.S. Department of Transportation Drug and Alcohol Testing MIS Data Collection Form. A vessel must be provided with bilge pumps in accordance with Table 182.520(a). Vessels 1 fixed power pump is not operational. 46CFR182.520(a)

Vessels pre-engineered (Fireboy) fire extinguishing system protecting the machinery space does not shut down the main engine and does not meet the minimum standard of automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space in accordance with 46 CFR 181.420(a)(3). The vessel is restricted from carrying passengers until corrected to the satisfaction of an attending Coast Guard Marine Inspector.

****VESSEL RESTRICTED TO DAYLIGHT OPERATIONS ONLY ***** The vessel currently is utilizing an all around light for the stern light. Install a stern navigation light with an arc of visibility of 135 degrees.

Provide a suitable first aid kit that is either approved under CG Approval 46 CFR 160.041 or ensure the first aid kit has all the required items as listed in the approval. The vessel will be restricted from carrying passengers after August 13, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels has several dead end wires (Starboard side aft, red wires in the console) and does not meet the standard of minimizing accidental contact with electrical installations per 46CFR183.200(c) . The vessel will be restricted from carrying passengers after August 13, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels machinery space ventilation does not have a closure device and does not meet the minimum standard of provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent as described in 46CFR182.465(h). The vessel will be restricted from carrying passengers after August 13, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels hose clamp raw water port side is not double clamped and does not meet minimum standard as described in 46 CFR 182.430(e). Owner must ensure nonmetallic hoses used are double clamped. The vessel will be restricted from carrying passengers after August 13, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels nonmetallic Lube oil hose leading in to oil filter port side on port side is degraded and does not meet minimum standard as described in 46CFR182.720(e). Owner must replace hose with materials as described in the above site. The vessel will be restricted from carrying passengers after August 13, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Port side engine room Lighting fixture did not have a have a guard and has dead end wires. The vessel will be restricted from carrying passengers after August 13, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel has 2 holes in the forward machinery space bulkhead that will allow the pre-engineered (Fireboy) fire extinguishing system upon discharge and does not meet the provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system as described in 46CFR182.465(h). The vessel will be restricted from carrying passengers after August 13, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel must identify and repair source of oil leaks that is leading to an excessive accumulation of oil in the vessels bilges as required by 46CFR176.830(b)(2). After the source is repaired, vessel must clean bilges and replace discharge hose to the electrical submersible pump to prevent accidental discharge of oily mixtures. Vessel representatives must ensure that oily mixtures are retained on board in the vessels bilges and only discharged to a reception facility as described in 33CFR155.330, this will be facilitated by disabling the automatic feature on your electrical submersible pump. The vessel will be restricted from carrying passengers after August 2, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels fire buckets did not have long enough lanyards attached to the handle in accordance with 46 CFR 181.610. The vessel will be restricted from carrying passengers after August 13, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Identify source and remove the accumulation of oil IAW 46CFR 176.830(b).

Vessel hull has blisters on both Port and Starboard Side. Ensure proper repairs by draining all blisters, rinse with fresh water and let completely dry. Ensure that damaged are is grounded away, cleaned and coated with fresh resin as described in NVIC 8-87 to the satisfaction of an attending Coast Guard Marine Inspectors.

Vessel water lubricated bearings on at the shaft hull penetration was completely un-seated and excessively worn on the viewable portions bearings of the and does not meet the minimum requirements that Rubber bearings must be renewed when any water groove is found to be half its original depth as required by 46CFR176.670. Replace Bearing. (a) and MSM VOL II SEC B CH 3 D 8.

Make all valves with through hull penetrations available for inspection per 46CFR171.119(b). Unable to verify 2 valves Port Side.

Vessel will fail to undergo a annual inspection by 19JUN2019 which is past its 3 month anniversary date of 19MAR2019 as required by 46CFR176.500(b)(1). Vessel is restricted operating with passengers to the satisfaction of the attending Marine Inspector.

Provide a means to manually activate the pre-engineered fixed gas system to the satisfaction of CG MI prior to carrying passengers. 46CFR181.420(a)(2).

Vessels bilge has excessive accumulation of oil that may create a fire hazard per 46CFR176.830(b).Identify and remedy prior to carriage of passengers.

Prior to carrying passengers, repair and demonstrate proper operation of the bilge high level alarm, visual indicator in accordance with 46 CFR 182.530 to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels ring life buoy is damaged and needs to be replaced per 46CFR180.70(a)(2) and ensure marking per 46CFR185.604(i) prior to carrying passengers. Cleared on the spot.

Vessels floating light is not activating and need to be replaced. Ensure watertight meets 46CFR180.70(d). Vessel is restricted from nighttime operations until replaced. Deficiency, if not addressed prior to the allotted due date become prior to carriage of passengers.

Provide crew training at least every 3 months as to the duties the crew member is expected to preform including tiller operation. Crew training shall be logged or documented with the date and general description of drill prior to 12APR18. Deficiency, if not addressed prior to the allotted due date become prior to carriage of passengers.

Remove all unused, dead-end or out of service wiring onboard to the satisfaction of the CG MI no later than 12APR18. 46CFR183.200 Deficiency, if not addressed prior to the allotted due date become prior to carriage of passengers.

Provide at least one crewmember that holds a Marine Radio Operators Permit on each vessel prior to 12APR18. Restricted to 1000 feet from shore. 47CFR80.159. Deficiency, if not addressed prior to the allotted due date become prior to carriage of passengers.

Provide at least one crewmember on each vessel with CPR/First Aid training per NVIC 1-91 no later than 12APR18. Deficiency, if not addressed prior to the allotted due date become prior to carriage of passengers.

Vessels emergency instructions do not address emergency tiller operations as required by 46CFR185.510(b). Create detailed emergency instructions for tiller operations no later than 12APR18. Deficiency, if not addressed prior to the allotted due date become prior to carriage of passengers.

The vessel owner/operator was in violation of 46 CFR 176.500(b)(1) when he failed to have the DOLPHIN V inspected within 90 days of the anniversary date. The current COI was issued on March 19, 2015 and the owner did not contact the Coast Guard for inspection until July 5, 2016. The annual inspection was required no later than June 19, 2016.

Demonstrate proper operation of the stern navigation light.

Properly install and demonstrate proper operation of the port and starboard side running lights. The running lights are currently in a combination configuration which is only permitted for a vessel of not more than 40 feet.

Properly install lagging around all exposed pipe in way of the exhaust from the main engine to the point where the cooling water is introduced.

Provide currently corrected charts and navigational publications for the area of operation.
