FIre fighting systems and appliances shall be in good working order and readily available for immediate use. PSCO observed the foam analysis report was expired on June 12, 2020.

The tank vessel must install an isolation valve capable of manual operation. It must be located at the vessel vapor connection and must clearly show whether the valve is in the open or closed position via an indicator, valve handle or valve stem. PSCO observed both the starboard and port side vapor collection line return valve to be inoperable with missing handle/stem.

Resolution: Verified the replacement of the valve handle for VCS piping isolation valve.
