The safety management system shall be maintained in accordance with the provisions of the International Safety Management Code.
PSCO attended vessel after a report of a discharge of oil while bunkering. Company provided bunkering safety checklist
(SMS.FM.004.e) which require when tank contents are approaching their upper level, the rate of filling should be reduced. Chief Engineer stated they did not request to reduce speed of transfer.
In the event of a tank overflow, all bunker should be stopped immediately and should not be restarted until the fault has been rectified and all hazards from the release have been eliminated. After tank overflow was discovered, crew did not stop operations and continued bunkering.
Per ship's Safety Procedures Manual - Sec 6, Master or the person designated by Master should record the crew daily resting hours in the format recognized by the flag state administration taking into account the guidelines of the MLC 2006. Daily crew resting hours have not been logged for the month of April.
Based on the performance of the crew as substantiated by the objective evidence cited above, an additional external verification is required to ensure ship’s procedures are followed onboard. In addition, training is to be conducted on ship's emergency response procedures for pollution incidents and bunkering operations.
