PSCO observed that the fire control plan was out of date, and was not matching applicable regulations. Symbology on the fire control plan did not match physical locations for equipment and door types; and lastly the electronic fire control plan was a different version than the physical copies located throughout the ship.

Vessel failed to report deficiencies 01 through 04 in accordance with TGM 2.1.1 of section 4.3 of the vessel's safety management system.
