VSL HAD A HOLDING TANK FOR AN MSD AND RECENTLY CLEANED THE SYSTEM. AFTER CLEANING THE DIRECT DISCHARGE VALVE WAS LEFT IN THE OPEN POSITION. A CREWMEMBER FLUSHED THE TOILET AND I OBSERVED THE SYSTEM DISCHARGE INTO THE WATER. THE MASTER STATED THAT THE TANK WAS CLEANED SIX DAYS EARLIER.
