A Class-1 door may be installed at any place if...the vessel’s pilothouse contains a visual indicator showing whether the door is open or closed. The indicator for both water tight doors would not operate properly due to the indicator lever not being contacted properly by the door mechanism. Make proper repairs to ensure complete contact is made each time the door is closed.

Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the SMI. (i) The alcohol testing of each individual must be conducted within 2 hours of when the SMI occurred, unless precluded by safety concerns directly related to the incident. When vessel is offshore on a more than 2 hour voyage, no capability to conduct alcohol testing was available. Testing strips onboard were expired. Provide valid testing capability prior to conducting voyages of more than two hours from any capable testing location but no later than 14 days.

Vessel must have life buoys with at least one-half the total number of lifebuoys, but not less than two, be fitted with a self-igniting light approved under approval series 161.010. The lights attached required (based on manufacturer requirements) energizer lithium-ion batteries. However, all lights were using alkaline batteries. Replace all batteries with those required by the manufacturer no later than 14 days.
