Every company should develop, implement, and maintain a safety management system which includes the following functional requirements: instructions and procedures. SMS cybersecurity plan states "Master must ensure login password are not printed on PC (remove the notes or print outs)." Vessel failed to implement its SMS with respect to cyber risk management. Vessel posted username and password at multiple terminals. Note: Vessel must conduct internal audit to ensure SMS cyber hygiene is properly implemented.

All ships of 300 gross tonnage and upwards and passenger ships irrespective of size shall, in addition to meeting the requirements of paragraph 2.2, be fitted with: a 9 GHz radar, or other means, to determine and display the range and bearing of radar transponders and of other surface craft, obstructions, buoys, shorelines and navigational marks to assist in navigation and in collision avoidance. Vessel report that their X Band radar was inoperable.
