74 SOLAS (14)II-2/4.1.1 Means shall be provided to control leaks of flammable liquids. PSCO observed an active F/O leak on the No. 1 generator. 17ac changed to 15ac (see narative)

74SOLAS (14) / CH IX / 3.1 ISM Code (2018) / 9.2 The company should establish procedures for the implementation of corrective action, including measures intended to prevent recurrence. Vessel was issued deficiency by USCG for F/O leaks on the No. 1 and No.2 generators on 17 Jan 2021. Deficiency was rectified by DNV-GL survey statement # 1550929 on 17 Jan 2021 stating the crew arrested cause of the fuel oil leaks. On 18 Jan 2021 vessel submitted report of non-conformities to V Ships Hamburg GMBH and Co KG. Under "NC 02" which addressed the generator fuel oil leak, no preventative action were identified. Recommend external audit of V ship Hamburg GMBH and Co KG Safety Management System for adherence to and verification of implementing corrective and preventative actions. 50c

04114 - Emergency source of power - The electrical power available shall be sufficient to supply all those services that are essential for safety in an emergency. PSCO observed quick closing valve for the emergency fuel shutoff in the closed position and questioned the operational readiness of the emergency generator. 74 Solas (04) 11-1/43.2 10c

07124 - Fire protection systems shall be kept in good order as to ensure their required performance if a fire occurs, including structural fire protection and fire resisting divisions. The "A" Class bulkhead separating the galley and the stairwell was discovered held together with duct tape on the corners and holding wall panels to the bulkhead. PSCO questions the effectiveness of this boundary if a fire were to occur. 74SOLAS(14)II-214.2.1.1

07126 - Means shall be provided to control leaks of flammable liquids. PSCO observed active fuel leaks on the No. 1 and No. 2 generators in the vicinity of the fuel pumps and lube oil leaks w/ fuel accumulation in the drip pans and a significant amount of lube oil on the deck below the No. 2 generator. Oil soaked lagging was located in the purifier room on the No. 1 and No. 2 HFO purifiers. The No. 2 HFO purifier also had missing lagging on the fuel line. A small pump in the purifier room near the No. 1 HFO purifier has oil accumulation in the drip pan. 74SOLAS(04)II-2.4.1.1

07199 - Flammable liquid lockers shall be protected by appropriate fire extenguishing arrangement approved by the administration. Fuel samples are being stored in the steering gear room where fixed fire protection does not exist. 74SOLAS(04)II-210.6.3.2.

PSCO Observed that the IOPP Form A list that the vessel has a MEPC60(33)oil content meter installed, but upon inspection of the oil content meter it was discovered that the vessel has a MEPC 107(49) content meter installed. MARPOL Annex 1 Reg 6.1.2 MEPC 107(49) ACTION Taken: 50ac
