Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the oil record book part 1. PSCO found an entry dated 04AUG2022 that stated the ows exceeded the maximum throughput of the system. After review of the ORB part 1, IOPP, Engineering and sounding logs, and OWS records, PSCO was able to determine the entry was improperly recorded in the ORB part 1.

Machinery, boilers, and other pressure vessels, associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended. Starboard seawater pump that services the cargo oil pumps was found leaking water from the packing gland, creating a large collection of water below the pump. crew attemped to tighten packing, and overheated gland, to the point where steam was observed. PSCO is unable to verify adequate cooling can be provided to the cargo oil pumps, and the flow rate from leak from this pump. Vessel is able to provide adequate cooling to the condenser from two other pumps of adequate capacity.

To prevent the ignition of combustible materials of flammable liquids, means shall be provided to control leaks of flammable liquids. Oil was found leaking from valve one pipe coming from the Filter Drain Tank and fuel oil supply valve for #3 generator, overflowing makeshift collection containers, and draining down into the bilge.

After any survey of the ship has been completed, no change shall be made to equipment or other items covered by the survey, without the sanction of the administration. Engine control room air conditioner seawater discharge had makeshift flexible hose (designed for use in air systems) running to the inert gas scrubber seawater discharge.

Special Consideration shall be given to the design and locatin of scrubber and blowers with relevant piping and fittings in order to prevent flue gas leakages into enclosed spaces. Bearing seal on #2 inert gas blower was found to be leaking (i.e. not creating a gas tight seal).
