Repairs to machinery or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. On 11JUL26 vessel reported a pinhole leak (aprox 1-2mm in size) on the firemain. Make temporary repair to the satisfaction of the attending marine inspector. Submit repair plan and make permanent repairs within 5 days (16JUL26). Vessel shall satisfactorily test firemain prior to each transit to ensure proper condition of temp repair as well as increase rounds of machinery space.

Each vessel must carry at least one rescue boat. Vessel is unable to retrieve the rescue boat without bypassing davit controls. Repair system and demonstrate operation to attending marine inspector.

Each vessel must be provided with a visual/audible alarm at the operating station to indicate a high water level in each space with a non-watertight closure , such as a space with a non-watertight hatch on the main deck. Stbd aft cardeck flooding alarm was inoperable. Repair cardeck alarms and demonstrate operation to atttending MI.

Grab line on rescue boat was not properly secured. Glue/adhesive was separated along the starboard side during boat launch, causing line to droop into the water when deployed.

No repairs or alterations affecting the safety of the vessel with regard to the equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel reported the rescue boat's forward bridle attachment point on the starboard side had hair line cracks in the fiberglass and the fitting was lifted off the deck by .041 inch. Conducted satisfactory weight test of rescue boat test for a 4 person capacity. Vessel reported new lift sling for rescue boat as unsatisfactory, and was unable to conduct weight test. Vessel authorized to continue operations with previous sling and deck fittings. Rescue boat is restricted to a 4 person capacity and shall not be lifted from cradle unless for emergency operations. Replace lift sling and demonstrate weight test to satisfaction of attending Marine Inspector by 14MAR2025.

No repairs or alterations affecting the safety of the vessel with regard to the machinery, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel reported alarm for number 8 cylinder on Port main engine. Repair port main and demonstrate operation to attending marine inspector.

The managing operator reported damage to the #3 Generator that renders further use of the item unsafe until repairs are made. Make permanent repairs to the effected Generator and demonstrate to the satisfaction of attending CG Marine Inspector.

On a vessel of more than 65ft in length, the public address system must be a fixed installation and audible during normal operating conditions. The installed PA system was not audible outside of the house. Prior to credit drydock, demonstrate proper operation of the PA system to the satisfaction of the attenidng MI. 46 CFR 121.610.

A door in a main vertical zone bulkhead must be of the self-closing type. The door to the galley and the double doors to the mess did not latch closed. Prior to credit drydock, demonstrate proper operation of the firescreen doors in the mess/galley. 46 CFR 116.435.

Each ring life buoy on a vessel must be approved in accordance with 160.050. The grab line shall be 3/8in diameter polethylene or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. 03 liferings had frayed and deteriorated grab lines. Replace liferings prior to completion of credit drydock. 46 CFR 117.70.

Repair #3 Generator and demonstrate to the satisfaction of attending CG Marine Inspector. 46 CFR 120.310

46 CFR 121.620 The vessel is required to have two means of controlling engine speed. The second means of control is the center console operating station, which is not operating as designed. The vessel is authorized a one-time voyage from Hollis, AK to Ketchikan AK. Prior to departing Ketchikan, AK on 12 June, prove proper operation of controls to the attending Marine Inspector.

STBD engine experienced a Crankcase overpressure engine shut down. Cause of shut down is believed to be a faulty sensor. Vessel will have Caterpillar engine Tech. trouble shoot system and remedy problem. Vessel is required to ensure engine safety features continue to operate in a safe, reliable manner and must demonstrate proper operation of propulsion engine to the satisfaction of attending CGMI. 30Sep20: Sensor was tested and found Sat. However, during trouble shooting the #2 cylinder head was found cracked. Head was renewed a satisfactory underway operational test was conducted. Vessel was given permission to sail. On voyage to Ketchikan the crankcase pressure was steadily rising and CC breathers were abnormally fouled. Vessel is required to continue trouble shooting and correct problem prior to using engine while carrying passengers.
