Repairs to machinery or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. On 11JUL26 vessel reported a pinhole leak (aprox 1-2mm in size) on the firemain. Make temporary repair to the satisfaction of the attending marine inspector. Submit repair plan and make permanent repairs within 5 days (16JUL26). Vessel shall satisfactorily test firemain prior to each transit to ensure proper condition of temp repair as well as increase rounds of machinery space.

No repairs or alterations affecting the safety of the vessel with regard to the equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel reported the rescue boat's forward bridle attachment point on the starboard side had hair line cracks in the fiberglass and the fitting was lifted off the deck by .041 inch. Conducted satisfactory weight test of rescue boat test for a 4 person capacity. Vessel reported new lift sling for rescue boat as unsatisfactory, and was unable to conduct weight test. Vessel authorized to continue operations with previous sling and deck fittings. Rescue boat is restricted to a 4 person capacity and shall not be lifted from cradle unless for emergency operations. Replace lift sling and demonstrate weight test to satisfaction of attending Marine Inspector by 14MAR2025.

STBD engine experienced a Crankcase overpressure engine shut down. Cause of shut down is believed to be a faulty sensor. Vessel will have Caterpillar engine Tech. trouble shoot system and remedy problem. Vessel is required to ensure engine safety features continue to operate in a safe, reliable manner and must demonstrate proper operation of propulsion engine to the satisfaction of attending CGMI. 30Sep20: Sensor was tested and found Sat. However, during trouble shooting the #2 cylinder head was found cracked. Head was renewed a satisfactory underway operational test was conducted. Vessel was given permission to sail. On voyage to Ketchikan the crankcase pressure was steadily rising and CC breathers were abnormally fouled. Vessel is required to continue trouble shooting and correct problem prior to using engine while carrying passengers.
