Each item of life saving equipment determined by the MI to not be in serviceable condition must be repaired of replaced. MI found the forward life ring to be faded and worn, replace. 46 CFR 180.70

A visual and audible alarm must be provided at the operating station to indicate a high water level in machinery spaces. The visual indicator was always illuminated. Audible alarm functioned properly. 46 CFR 182.530

Electrical equipment must be installed and maintained to protect passengers and from electrical hazards such as fire. MIs observed drop cords and electrical strip style surge protectors for lighting systems and drink dispersal in the forward stores and fuel tank room. Remove and replace with permanent installations. 46 CFR 183.200

a vessel more than 65 feet must carry a rescue boat unless the OCMI determines that the vessel is able to act as its own rescue vessel and the recovery of a helpless person can be observed from the operating station. The master has no direct line of sight from the operating station. Vessel must devise a plan to allow for the master to observe the recovery. 46 CFR 180.210

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Marine Inspector identified engine room bilge suction was unable to draw a suction during operational test of the system. Troubleshoot problem and show proper operation of bilge system from all compartments with suction points. 46 CFR 182.500(a)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. Marine Inspector identified bilge high level float alarm that was missing parts holding float in place and mounted horizontally in aft engine room causing it to not work properly. 46 CFR 182.530(a)(2)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Marine Inspector identified stbd ventilation closure for the engine room was not closing completely when actuated. Unable to test port side due to inaccessibility. Repair and show proper operation of both port/stbd ventilation closures for the engine room. 46 CFR 182.460(i)

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency. Marine Inspector reviewed training log and found that the latest drills were conducted 7/2023. Previous drills had been conducted 4/2022. Talking with new Captain and crew that they were unsure what was expected during drills. All satisfactory drills are to be completed in the presence of CG Marine Inspector. 46 CFR 185.520(a) 46 CFR 185.524(a) 46 CFR 185.420(c)

A hatch exposed to the weather must be watertight. Identified watertight hatch located stbd side for the steering compartment that is not sealing properly alloying water intrusion into the space. 46 CFR 179.360(a)

A vessel must be fitted with a Federal Communications Commission (FCC) type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station if: (2)The vessel has an oceans, coastwise, limited coastwise, or Great Lakes route; and (3) The vessel carries more than 49 passengers. Radar was found to be inoperative due to part failed linking the radar unit with the exterior mounted antenna unit. Reduced route to lakes, bays and sounds route. 46 CFR 184.404(a)(2)(3)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. All alcohol tested swabs onboard were expired. Alcohol testing swabs that are not expired must be on board unless Captain can produce alternate means to test individuals within the required timelines. 46 CFR 4.06-15(a)(1)

When a marine employer determines that a casualty or incident is, or is likely to become, a serious marine incident, the marine employer shall take all practicable steps to have each individual engaged or employed on board the vessel who is directly involved in the incident chemically tested for evidence of drug and alcohol use as required in this part. Found that the newly hired Captain was not completely aware of the process(s) to be done during serious marine incident as per the drug consortium requirements/regulations. Captain must show knowledge of the drug consortium and its particulars/requirements. 46 CFR 4.06-1(b)

Vital systems are those systems that are vital to a vessel's survivability and safety. Marine Inspector identified leak in discharge piping from the fire pump passed the gauge right side of piping passed valve leaking at threaded joint. 46 CFR 182.710(a)

Marine Inspector identified stbd main engine raw water piping from sea strainer to engines raw water pump that could be turned by hand at the elbow joint. 46 CFR 182.710(a)(6)

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Captain could not provide a 3rd fire technian servicing report for portable extinguishers and the fixed-gas fire extinguishing system. **Provide report within seven days of the technicians report and what they conducted tests on.** 46 CFR 176.810(b)(2)

The name of the vessel must be marked or painted in clearly legible letters on each Emergency Position Indicating Radiobeacon (EPIRB). Identified EPIRB did not have the name of the vessel marked on it. *Corrected on the spot. 46 185.604(c)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. The vessel has a COI anniversary date of 05APR. The vessel was required to have an annual inspection no later than 05JUL23, and the vessel has not had an annual inspection for this year. 46 CFR 176.500(b)(1)

Fireplaces or other space heating equipment with open flames are prohibited from being used on all vessels. Identified burners used for heating chaffing dishes. Crew found proper electrical heating elements for the chaffing dishes. *Corrected on the spot.* 46 CFR 184.202

Fuel lines must be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft nonferrous metal straps which have no sharp edges and are insulated to protect against corrosion. Identified fuel hose leading from the racor filter to the high pressure fuel pump that was chaffing on the starboard main diesel engine. 46 CFR 182.455(b)(3)

Water for cooling the exhaust pipe, other than a vertical exhaust, must be injected into the exhaust system as near to the engine manifold as practicable. The water must pass through the entire length of the exhaust pipe. Identified saltwater leaking around the fiberglass exhaust bubbler on the port main diesel engine. 46 CFR 182.425(b)(2)

Fireplaces or other space heating equipment with open flames are prohibited from being used on all vessels. Sterno open flame heating was being used under warming food trays. 46 CFR 184.202(b)

Each ring life buoy on a vessel must be approved in accordance with 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 161.150. Main deck, aft ring buoy found to have wasted outer cover exposing interior buoyancy material. Deemed unserviceable. 46 CFR 180.70(b)(1)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. Found 110 adult life jackets to have deteriorated retroreflective material that will not last till the first annual inspection. 46 CFR 185.604(h)

Repair two areas of low readings, approximately 8'7 1/2" X 17 1/2" of bottom plate, port/starboard, in way keel, third compartment aft of bow, below the fuel tank.
