Door handle/lock on boat deck portside house was found broken and unable to secure.

Stairways and lift trunks which penetrate more than a single deck shall be surrounded by at least "A-0" class divisions and be protected by self closing doors at all levels. MI observed the door at the stairway on the upper deck level was not properly self closing. 74 SOLAS (14) II-2/9.2.3.4.1

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. MI observed multiple oxygen and acetylene cylinders resting on the deck, instead of in racks and without their caps in place. 46 CFR 147.60(b)

The jacketed [fuel] piping system shall include a means of collectioin of leakages and arrangements shall be provided with an alarm. Feul oil leak alarms were not functioning as designed. Crew was aware of condition and silenced alarms for a prolonged period of time. 74 SOLAS (2014) II-2/4.2.2.5.2
