Emergency lighting from the engine room to the mess deck were found to be insufficiently bright. Additionally the emergency light at the base of the ladder leading into the corridor to the engine room was non-functioning and in need of repair.

Offshore supply vessels less than 100 gross tons shall had rudder-angle indicators installed at the control station in the pilothouse. Rudder-angle indicators at the forward and aft control stations in the pilothouse were found unable to indicate rudder angles beyond 30 degrees despite rudders being capable of 45 degrees. Additionally, the pins attaching the rudder arms to the hydraulics were excessively worn and in need of replacement.
