A secondary means of allowing full flow relief of vapour, air or inert gas mixtures shall be provided to prevent over-pressure or under-pressure in the event of failure arrangements. PSCO observed that the liquid level in P/V breaker was lower than what was required due to the pressure in the main IG line.

The design, testing and location of these devices shall comply with the requirements established by the Administration based on the guidelines developed by the Organization. PSCO observed wrong flame screens installed on WBT tanks.
