Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Light fixture in laz was broken. Fix light fixture and notify local OCMI. 46 CFR 183.410

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found in laz and 03 DCV’s in the engine room were missing plates and showing exposed wires. Remove dead end wiring and update electrical plans. 46 CFR 183.200 (c)

An alarm for a fixed gas fire extinguishing system must be clearly and conspicuously marked “WHEN ALARM SOUNDS-VACATE AT ONCE. CARBON DIOXIDE BEING RELEASED”. Where a different extinguishing agent is installed, that agent shall be marked in place of “carbon dioxide.” Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”. (2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. 46 CFR 185.612

Each vessel must have adequate emergency lighting fitted along the escape to the main deck from all passenger and crew accommodation spaces located below the main deck. 03 emergency lights in the passenger accommodation were inoperable. Replace emergency lighting and notify local OCMI. 46 CFR 183.432

Each ring life buoy on a vessel must: Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Mi found port and stbd ring life buoy to be no longer serviceable. Replace ring life buoy. 46 CFR 180.70

Vessel's port propeller struck a submerged log while underway. Provide servicing report of repairs and balancing of the port propeller prior to carrying passengers.

Remote fuel shutoff valve failed during testing. Repair/replace the fuel shutoff valve and demonstrate operation to the attending Coast Guard marine inspector. Cite: 46 CFR 182.455(b)(4)

There was no fire detector in the engine room. Install an integrated fire detector in the engine room and demonstrate operation to the attending Coast Guard marine inspector. The detector must be in compliance with 46 CFR 181.450. Cite: 46 CFR 181.405(c)

Vessel reported that its starboard shaft is cracked and is causing a vibration when run. Vessel must repair or replace the affected shaft.

The Laz hatch was found to be missing the gasket. Make hatch water tight.

The ER escape hatch was found to be inoperable from the inside. Provide means to operate escape hatch from inside the compartment.

Extension cord cable was found wired to a light fixture on the main deck port side closet. Provide proper boat cable for the light fixture.

Vessel's steering was inoperable due to hydraulic ram maintenance. Prove proper operation of steering.

Ring buoy light was found inoperable. Prove operation of light.

Vessel was unable to get underway. Preform underway man overboard drills and provide proof to SSD.

03 ring buoys were missing retro reflective tape. Add retro reflective tape to front and back of all ring buoys at "four corners".

Vessel's EPIRB had an expired battery and could not be replaced. Replace and register the new EPIRB with NOAA. Vessel is restricted to no more than 3NM from shore until this deficiency is rectified.

The vessel's fire stations did not have spanner wrenches. Provide a spanner wrench at each fire station.

The vessel's fire pump gauge is stuck. Replace the gauge on the fire pump.

Electrical equipment must be installed and maintained to protect passengers, crew and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Correct the following: (1)Label all sub-panels (2)Provide blanks for missing breakers (3)Provide switchboard with latching/securing device. (4)Make permanent repairs to "main panel", make cover able to latch/close. (6)Make main panel free of any openings. (7)Remove surge protect @ TV (8) Properly wire and install outlets in crew cabins.

Complete but simple instructions for the operation of the fixed gas fire-extinguishing system must be located in a conspicuous place at or near each pull box and stop valve and in the space where the cylinders are stored. Crew member did not know location of E/R CO2 system remote pull. Label with "Engine Room CO2 remote pull station - On master's order - Pull handle - Cover ventilation - Close remote fuel shutoffs."

Provide additional ladder rungs above main deck on forward emergency escape. Mark two rungs below deck at hinge locations with "PULL"

Move "EMERGENCY EXIT - KEEP CLEAR" markings closer to emergency exit hatch in galley.

Repair escape plunger in starboard freezer.
