Additionally, escape route signs and fire equipment location markings shall be of photo luminescent material or marked by lighting. PSCO obseived low location lighting erroneously indicating a door on deck 3, starboard side aft, exit by the dining room entrance. Additionally, PSCO o bsetved low location lighting erroneously indicating a door o n deck 3, port side, by the mid ship stair.

The vessel's FSD 02.4.41 damage to bottom hinge prevented door from closing. FSD 04.6.10 hit door jam at the bottom and did not close properly.
