Muster and embarkation stations shall be adequately illuminated by lighting. PSCO observed at least 3 sets of embarkation lightings not illuminated. 74 SOLAS 20 III/11.4

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Port rescue boat did not start upon actuation of engine. 74 SOLAS 20 III/20.2

Escape doors from public spaces that are normally latched shall be fitted with a means of quick release. Such means shall consist of a door-latching mechanism incorporating a device that releases the latch upon the application of a force in the direction of escape flow. PSCO observed thumb type latches on multiple escape doors deck 10 child area. 74 SOLAS 20 II-2/13.3.6.2

A fixed fire-extinguishing system, required by this chapter shall comply with the provisions of the Fire Safety Systems Code. In accommodation and service spaces the sprinklers shall come into operation within the temperature rance fom 68 C to 79 C. PSCO observed empty ampule on sprinkler head, ivo section valve 11,16,17 station. 74 SOLAS 20 II-2/10.4.1.1

Ignition sources shall be restricted; In electrical locker 10.3.20, PSCO observed open paint cans, rags and other painting debris left unattended, with no worker nearby. 74SOLAS20 II-2/4.1.4 17c

Cylinders must always be secured, and when not in use, they must be stowed in a rack... PSCO observed 12 nitrogen and refrigerant gas bottles secured to railing (engine room) that is not designed as a securing point for compressed gas bottles. 46CFR147.60(b)

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and liferaft embarkation deck. For this purpose, the following functional requirements shall be met: .1 safe escape routs shall be provided; Behind FSD 08.4.26, PSCO obsereved a closet storing houseware that prevented the entry/exit door from opening all the way, additionally the path thughout the space was severely obstructed with goods. 74SOLAS 20 II-2/13.1.1

Parties responsible for fire extinguishing shall be !Organized. These parties shall have the capability to complete their duties at all times while the ship is in service. A member of Attach Fire Team 1, as observed during the drill, ??dentified as having low-flow to his SCBA, after entering the !fire area. 74 SOLAS 2011-2/15.2.1 3

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation,or where it is possible to assign two or more classifications to a space, it shall be treated as a space within the relevant category having the most stringent boundary requirements. NC machinery spaces 1/1.01, 2/4, and 1/2 iare identified as category (I 0) spaces, and were found to have combustibles stowed in the spaces. 74 SOLAS 20 ll-2/19.2.2.3.2.2

Fire doors in main vertical z.one bulkheads, galley ooundaries and stairway enclosures shall be self-closing and be capable of closing with an angle of inclination of up to 3.5° opposing closure. PSCO observed FSD 1.1.69, FSD 04.7.03, FSD 04.6.05, FSD 3.1.82 would not close as intended. 74SOLAS2011-2/9.4.1.1.5.l

Cabin doors in "8" class divisions shall be of a self-closing ??pe. Hold-back hooks are not pennitted. Three crew bibins, doors 8096, B 110, and 8098 were observed to not be self closing. 74SOLAS20 II-2/9.4.1.2.2

The following functional requirements shall be met: escape routes shall be maintained in a safe condition, clear of obstacles. The passageway near room A.3.09 was Mentified as an esacpe route, and was incorrectly being used to stow luggage and other supplies. 74 SOLAS 20 11-2/13.1.2

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Four (4) stairway guides in crew stairways forward 7,8,9 and aft 6 were not familiar with ship's emergency procedures for passengers returning to their cabins during an abandon ship scenario.

Waste receptacles shall be constructed of on-combustible materials with no openings in the sides or bottom. PSCO obsetved two garbage cans in galley areas n deck 1 and 3 where combustible waste was stowed in plastic garbage cans. 74 Solas 20 II-2/4.4.2

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers/crew to identify the outes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photo luminescent material or marked by lighting. PSCO obseived low location lighting erroneously indicating a door on deck 3, starboard side aft, exit by the dining room entrance. Additionally, PSCO o bsetved low location lighting erroneously indicating a door o n deck 3, port side, by the mid ship stair.

In interior locations in passenger ships carrying more than 36 passengers, fire hoses shall have necessary fittings and tools for use kept in conspicuous postilions near hydrants. the hydrant located as location 01.01 .29 was missing a spanner wrench. 74 SOLAS 20 II-2/10.2.3.1.1

The following functional requirements shall be met escape routes shall be maintained in a safe condition, clear of obstacles. Following stairwells have items obstructing means of escape: 1) Crew stairwell; deck 09, MVZ 4, tables are stored 2) Passenger stairwell, signs stored near spa on deck 09. 3) Passenger stairwell, carts stored on deck 09, MVZ 4. 4) Charis on deck 09 ivo Ledo deck restaurant. 74 SOLAS 20 II-2/13.1.2

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. Fire hose station (9.1.27) is secured shut with tie down line, not easily accessed if required. 74 SOLAS 20 II-2/14.1.1

A fixed fire-extinguishing system, required by this chapter shall comply with the provisions of the Fire Safety Systems Code. PSCO observed 04 failed high-fog sprinkler heads on deck 8 & 9, in fire zone 06. Sprinkler heads (C10-57C70 1662, C10-57C/0 1662) failed to activate and discharge water upon application of heat, via a heat gun, on ampules. 74 SOLAS 20 II-2/10.4.1.1

A fixed fire detection and fire alarm system and sample extrication smoke detection system shall be of an approved type and comply with the Fire Safety Systems Code. Any required fixed fire detection and fire alarm system with manually activated call points shall be capable of immediate operation at all times. PSC observed 02 inoperable smoke detectors number 127-154 on deck 10 aft and 01 inoperable call point outside room 10004. 74 SOLAS 20 II-2/7.2.4.2

Emergency escape breathing devices shall comply with the Fire Safety Systems Code. Spare emergency escape breathing devices shall be kept onboard. PSCO observed an expired EEBD in Eengine Room, IVO FSD 0C.2.19. 74 SOLAS 20 II-2/13.3.4.1

Waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed plastic garbage cans with general combustible waste throughout the ship in galleys, pantries, and in the garbage handling room- where ship's crews were not actively sorting wastes. Per the ship's garbage management plan, crews are permitted to use combustible garbage cans in galleys, bars and pantries- this practice is in violation to the subject regulation.

Every ship of 100gt or above and every ship that carries 15 or more persons….shall carry a garbage management plan that the crew shall follow. Placard to indicate how to dispose of garbage is contradictory to Garbage Management plan. Plan indicates all garbage shall be disposed of in non-cobnustable recepticales, except for pantries, galleys and bars. ANNEX V / Reg 10.2

Fire extinguishers shall be situated ready for use at easily visible places, which can be reached quickly and easily at any time in the event of a fire, and in such a way that their serviceability is not impaired by the weather, vibration or other external factors. Portable fire extinguishers shall be provided with devices which indicate whether they have been used. PSCO observed a fire extinguisher 10.6.03, in a locker behind two doors in the crows nest lounge, without any signage indicating fire extinguisher is located there. 74 SOLAS 20 II-2/10.3.2.4

The construction of doors and door frames in 'A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated. PSCO observed considerable (aprox. 1"X1/4") gap in hose port on FSD 10.6.02.

Any ship of 10,000 gross tonnage and above shall be fitted with oil filtering equipment complying with paragraph 7 of this regulation. Oil content meter on static oily water separator, is activating the 3 way valve beyond the 15 ppm threshold. MARPOL 2017 Annex I / 14.2

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Stbd steering gear fall prevention railing has crack at based stanchion, considerably weakening rail holdback strength. 74 SOLAS 04 II-1/26.1

Fire hoses shall be of non-perishable material and shall be sufficient in length to project a jet of water to any of the spaces in which they may be required to be used Each hose shall be provided with a nozzle and the necessary couplings. Fire hose station D.03.13 and D.04.09 were missing spanner wrenches. 74 SOLAS 20 II-2/10.2.3.1.1

Where the contents and use of a space are such that there is adoubt as to its classification for the purpose of this regulation, or where it is possible to assign two or more classifications to a space, it shall be treated as a space within the relevant category having the most stringent boundary requirements. PSCO observed storage of combustible material (solids annd liquids) in technical spaces IVO crew cabin C001, C007, B003, C018, B120, B007, fire station B.4.18, B.05.07, B. 04.04, fire extinguisher C.04.02, and in AC space behind door 4.4.25. 74 SOLAS 20 II-2/9.2.2.3.2.2

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed a holdback device on an electrical panel in garbage room near escape ffrom machinery space. 74 SOLAS 04 II-1/40.1.3

Openings in "A" class divisions. Openings in bulkheads and decks in passenger ships. Fire Doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other other than power-operated watertight doors and those which are normally locked shall satisfy the following requirements: a door re-open not more than 1 m from the point of contact. The vessel's FSD 02.4.41 damage to bottom hinge prevented door from closing. FSD 04.6.10 hit door jam at the bottom and did not close properly. The vessel's FSD 02.5.18 had a cracked doorframe and the timing prevented the door from closing properly. 74 SOLAS 01 II-2/30

Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. Portable extinguishers which have been discharged shall be immediately recharged or replaced with an equivalent unit. Fire station cabinet missing access handle. 74 SOLAS 20 II-2/14.2.1.2

Every ship shall be provided with: subject to provisions of regulation 8.3, a satellite Emergency Position-Indicating Radio Beacon (EPIRB). EPIRB in free arrangement above bridge found with expired battery. SOLAS 74(2020) IV/7.1.6

In addition to the emergency lighting required by regulations II-I/42 and III/11.5, The means of escape, including stairways and exits shall be marked by lighting or photo luminescent strip indicators placed not more than 300 mm above the deck at all points of escape routing including angles and intersections. Electric low location lighting was extinguished in various locations throughout Deck 0 MVZ 5 to include the hospital and crew accommodation. SOLAS 20 II-2/13.2.5.1

Escape doors from public spaces that are normally latched shall be fitted with a means of quick release... Quick release mechanisms shall be designed and installed to the satisfaction of the Administration and in particular: not be equipped with any locking device, set screw or other arrangement that prevents the release of the latch when pressure is applied to the releasing device... Escape route doors in Deck 10 "Club Hal" had installed twist locks without means for quick release.

The traditional wooden lining on the bulkheads and ceiling are permitted in sauna. the distance from the hot surfaces to combustible material shall be at least 500mm or the combustible materials shall be protected. There was no protection on combustible material and charring was present in mens sauna. Solas Ch II-2 Reg 2.2.3.4.3

Every crewmember with assigned emergency duties shall be familiar with these duties before the voyage begins. Port side life boat team was unfamiliar with the decending units for life boats. Solas Ch III Reg 19 2.1

In addition to the emergency lighting required..., the means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicaters placed not more than 300mm above the deck at all points of escape routes. Low location lighting or tape is missing from aft crew stairway deck A & B. Solas Ch II-2 Reg 13 3.2.5.1

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with structural, mechanical and electrical requirements of a Class/RO or with applicable national standards of the Administration which provide an equivalent level of safety. Identified a piece of floor tile raised and not flushed with the surrounding floor presenting a tripping hazard to passengers and crew. Floor tile is located on the starboard side deck one, at the entrance to the Vista Lounge. The crew placed a yellow cone in the area for passengers to avoid the tile until repairs are complete. Solas 2002 Ch. II Reg 3-1

11112: Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The #5 lifeboat davit is not ready for immediate use. 74 SOLAS 14 CH. III 20.2

Passenger ships shall carry lifeboats on each side of such aggregate capacity as will accomodate not less than 50% of total persons on board. Lifeboat #13 unable to launch due to failure of davit. 74SOLAS(09 Cons) III/21.1.1.1.

Recovery time of rescue boat shall be not more than 5 min in moderate sea conditions. Stbd side rescue boat hydraulic slewing motor is inoperable with parts on order. Crew must use manual operation to cradle the boat increasing recovery time to greater than 5 min. 74SOLAS(09 Cons) III/17.4

Lifeboats which are not self-righting shall have suitable handholds on underside of hull and shall be fastened to the life boat. Lifeboat #13 port hand hold is missing bolt on fastener. 74SOLAS(09 Cons) III/34 LSA Ch. IV 4.4.7.4.

Above the bulkhead deck there shall be at least two means of escape from each verticle zone. Escape route within electrician shop blocked by installed shelf. 74SOLAS(01 Cons) II-2/28.1.2

Firedoors in galley boudaries shall be capable of remote release. Roller shutter in galley space unable to close. 74SOLAS(01 Cons) II-2/30.4.3

Buoyant material for rescue boats installed external to hull is adequately protected against damage. Stbd side rescue boat fender fixture line is chaffed against rescue boat hull. 74SOLAS (09 Cons) III/34 LSA Code Ch. V 5.1.1.2.

B Deck Emergency Escape leading to C & D deck does not have adequate fire suppression.

Fire door closures shall be no more than 40s and no less than 10s from the begininning of movement with the ship in an upright position. Fire door OC.5.13 closed in 1s.

Pantry P.3.6.02 escape route shall be maintained in a safe condition. Pantry exit blocked with crates.

Within the perimeter of such stairway enclodures, only public toilets, lockers of non-combustible materials providing storage for non hazardous safety equipment and open information couters are permitted. Stairwell #14 A-deck contained a bike rack with 6 bicycles stored.

Storage of combustibles is prohibited in air conditioning rooms. AC station 1.1 had combustible items stored.

Smoke detectors shall be installed in all stairways, corridors and escape routes within accomodation spaces as provided in paragraph 5.2, 5.3, and 5.4, consideration shall be given to the installation of special purpose smoke detectors within ventilation ducting. The smoke detector in the ship spa was bad.
