Flag State Dispensation is granted to the M/V MAUNALEI to operate in the Singapore region to conduct sea trials operational testing with five (05) additional persons, provided the following conditions are met: 1. Must remain within 20NM of the Singapore near coastal region. 2. Maximum of (27) total persons onboard. 3. An approved and suitable life jacket and immersion suit will be given to each person onboard. 4. All life saving appliances must be in good working order. 5. Living quarters and bathrooms are to be arranged and available to each person onboard. This is set to expire at the conclusion of sea trials or December 2nd, whichever is first. Vessel must notify our office at the conclusion of sea trials.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Vessels current trim renders the emergency fire pump inoperable and not ready for immediate use. 74SOLAS(20)II-2/14.2.1.2

Each liferaft must be arranged to permit it to drop into the water from the deck on which it is stowed, taking into account the height of the stowage location at maximum freeboard. All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. 74 SOLAS (04) II-1/26.3.10 Means shall be provided whereby normal operation of propulsion machinery can be sustained even though one of the auxiliaries becomes inoperative. Special consideration shall be given to the malfunctioning of the hydraulic, pneumatic, or electrical means for control in main propulsion machinery including controllable pitch propellers. Vessel reported an issue with the CPP seal system, which is causing a discharge of CPP oil (Kluberio RM 2-100) into the water at a rate of approximately 2 liters every 24 hours.

EPIRB hydrostatic release unit expiration date must be marked on unit. Provide evidence that unit is within expiration date and mark HRU as such.

The general emergency alarm system shall be supplemented by either a public address system complying with the requirements of paragraph 7.2.2 of the Code or other suitable means of communication. Vessel self reported inoperable public address system. Vessel shall follow risk mitigation plan until rectified.

IN LIEU OF THE REQUIREMENTSOF PARAGRAPH 1.1, CARGO SHIPS MAY CARRY: ONE OR MORE INFLATABLE OR RIGID LIFERAFTS COMPLYING WITH THE REQUIREMENTS OF SECTION 4.2 OR 4.3 OF THE CODE, ON EACH SIDE OF THE SHIP, OF SUCH AGGREGATE CAPACITY AS WILL ACCOMMODATE THE TOTAL NUMBER OF PERSONS ON BOARD. THE LIFERAFTS ON AT LEAST ONE SIDE OF THE SHIP SHALL BE SERVED BY LAUNCHING APPLIANCES. THE 2- 16 PERSON LIFERAFTS THAT ARE REQUIRED TO BE SERVICED BY THE DAVIT ON THE PORT SIDE ARE NOT DAVIT LAUNCHED LIFERAFTS. VESSEL IS AUTHORIZED TO SAIL WITH THE LIFERAFTS ONBOARD FOR 30 DAYS. 74SOLAS(20)III/31.1.2.2 50ac

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. The vessel's lifeboat was damaged during launching and is not seaworthy. 46 CFR 91.25-15 46 CFR 199.190

All lifesaving appliances and arrangements shall comply with the applicable requirements of the code. The liferaft painter system shall provide a connection between the ship and the liferaft and shall be so arranged as to ensure that the liferaft when released and, in the case of an inflatable liferaft, inflated not dragged under by the sinking ship. The connection point between the liferaft and the ship was missing the pin on the port forward liferaft. SOLAS (20) III/34 LSA Code 4.1.6.1

All lifesaving appliances and arrangements shall comply with the applicable requirements of the code. Self-igniting lights shall be provided with a source of energy or a period of at least 2 hours. The port midships light was found to be inoperable. SOLAS (20)III/34 LSA Code 2.1.2.3

Record of emission sensitive components (ESC) replacement were not being maintained in the engine technical file. DNV provided guidance to use the vessel’s PMP/NSE. No part number recorded to validate the correct part was installed. Correct record to include required information. Correct procedures to include required information for future entries. Aeration pump removed from the CG approved Type II MSD and IMO approved sewage treatment system. No conditions of class noted. DNV issued SOVC requires IMO approved treatment system.

Fire protection insulation was removed in way of a cable penetration between the switchgear room and the space above and in the emergency egress route of the lower engine room. Replace structural fire protection IAW Fire Control Plan. SOLAS II-2/2.3.3

Scaling was noted in the overhead of the e-generator room. The overhead deck was examined and found wasted on the adjacent surface under the output of a deck drain. Boundary appears compromised. Repair as needed. SOLAS II-2/9

Paint locker CO2 system flex loops excessively loose where attached to the manifold. Have system properly serviced.

Each lifeboat shall be provided with an appliance which is capable of launching and recovering the lifeboat. Hydraulic hose on the recovery davit for the free-fall lifeboat was burst, needing replacement to operate as designed.

Seals on forward head tank ruptured, spilling oil at a rate of 4 liters per minute into the bilge. Vessel required to fix seals and ensure safe operation of head tank prior to next departure.

The fire integrity of the divisions shall be maintained at openings and penetrations. MTC in emergency generator room for newly installed shore tie cables missing insulation for appropriate fire boundary. 74SOLAS(04) II-2/9.1.3

Means of control shall be provided for stopping oil transfer pumps and oil seperators (purifiers). Vessel replaced #1 MGO purified in drydock and emergency shutdown outside the space in inoperable. Vessel restricted from using purifier until emergency shutdown is operational. 74SOLAS(04) II-2/5.2.2.3

Electrical Installations shall be such that the safety of the ccrew and ship from electrical hazards will be ensured. Witnessed daisy chain of electrical cords for CCTV system in switchboard room. 74SOLAS(14)II-1/40.1.3

Oil fuel pipes which if damaged would allow oil to escape from a storage, settling or daily service tank having a capacity of 500 L and above situated above the double bottom shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space. Vessel's pneumatic control station for quick closing valves had a check valve in the air supply to accumulator line that was inoperable. 74SOLAS(14)II-2/4.2.2.3.4

The starboard fuel tank (tank 5.05) has multiple sheared studs that allows marine diesel oil to "weep" out.

Crew could not adequately operate and launch rescue boat, and when attempting to bring rescue boat back in anti-two block switch engaged prematurely forcing crew to manually lift boat into cradle. Prove proper operation and timely deployment of rescue boat. 74SOLAS(14) III/20.2

For a period of three hours emergency lighting at every muster and embarkation station and over the sides must be provided. Emergency lighting at rescue boat embarkation and stbd bow life raft station not functional. Effect proper repairs and prove proper operation. 74SOLAS(04) II-1/43.2.1

Structural fire protection and protection of openings and penetrations in these divisions shall be kept in good order to ensure required performance if fire occurred. Multiple Cable Transit's in electrical switch gear rooms 613 and 113 as well as CO2 room are non-compliant due to unauthorized drilling and additional cable installations in existing compression blocks. Effect adequate repairs IAW MCT type approval. 74SOLAS(04) II-2/14.2.1.1.1

Ship must have fixed containment under or around each fuel oil or lube oil tank vent overflow / fill pipe. Secondary containment around stbd side aft fuel tank vents missing mechanical closures. Provide satisfactory closure for containment. 33CFR 155.320(a)

The company should clearly define and document the masters responsibility with regard to implementing safety and environmental protection policy of the company. Vessel personnel failed to utilize required confined space entry checklist as per SMS. 74SOLAS(14) IX ISM Code 5.1.1

The number of openings in watertight subdivisions is to be kept to a minimum compatible with the design and proper working of the ship. Where penetrations of watertight bulkheads and internal decks are necessary for access, piping, ventilation, electrical cables, etc., arrangements are to be made to maintain the watertight integrity. The Administration may permit relaxation in the watertightness of openings above the freeboard deck, provided that it is demonstrated that any progressive flooding can be easily controlled and that the safety of the ship is not impaired. All multiple cable transits (MCT) inspected were not installed IAW manufacture type approval, in both port and starboard side enclosed tunnels. All MCTs in watertight bulkheads throughout both port and starboard side enclosed tunnels are to be repaired and installation verified IAW manufacture type approval. Installation must include consideration of location of MCT as to the type approval water pressure rating. CG-835 to be completed in 60 days (29APR2019)
