Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Fuel valves on port and starboard tank did not have remote shutoff means. 46 CFR 182.455(b)(4)

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The pump outlet must be fitted with a pressure gauge. MI observed leak on the fire pump discharge elbow and 40 psi at the discharge gauge. 46 CFR 181.300(c)

Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. Galley fwd portable extinguisher exceeded 12 months inspection date.

Vapor barriers. Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Centerline exhaust piping penetration in aft bulkhead not vapor tight. 46 CFR 177.405(e)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). MI observed port life buoy missing retroreflective materiel. 46 CFR 185.604(i)

Except on a vessel limited to daytime operations, the compass must be illuminated. MI observed magnetic compass light inoperable. 46 CFR 184.402(c)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. MI observed anchor light as inoperable. 46 CFR 183.420

Pipes used for wet exhaust lines must be Schedule 80 or corrosion-resistant material and adequately protected from mechanical damage. MI observed port exhaust piping chaffing on deck longitudinal. 46 CFR 182.430(d)

Vessel Charts were not up-to-date. 46 CFR 184.420 "As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible"

Provide techinical specification sheet for air compressor to MI for review. Air compressor for ship's horn needs ASME approval. 46 CFR 54.10-20 Pressure vessels (replaces UG-116, except paragraph (k), and UG-118). Pressure vessels that are required by § 54.10-3 to be stamped with the Coast Guard Symbol must also be stamped with the following information: (1) Manufacturer's name and serial number. (2) Coast Guard number, see § 50.10-30 of this subchapter. (3) Coast Guard Symbol, which is affixed only by the marine inspector. (4) Maximum allowable working pressure ____ kPa (____ psig) at ____ °C (____ °F). (5) Class. (6) Minimum design metal temperature, if below -18 °C (0 °F). (7) Water capacity in liters (U.S. gallons), if a cargo carrying pressure vessel.

Pipe installation on a/c lines needs to be renewed.

A space containing propulsion machinery must have a fixed gas fire extinguishing system. Pre-engineered system was accidentally discharged during inspection. system must be serviced prior to carriage of pax. 46 CFR 181.400 Vessel has been authorized to carry passengers for one charter on 10 July. Vessel will provide 2 additional 40-BC fire extinguishers and 01 additional deckhand dedicated to engine room fire watch during this voyage. Upon completion of this charter, vessel is prohibited from carrying passengers until Pre-Engineered Fire Fighting system is recharged to the satisfaction of the Coast Guard.

A vessel must carry adaquate and up to date charts. Charts on board are dated 2017.

A vessel must carry a first aid kit approved under 160.041. First aid kit did contain ammonia inhalants. 46 CFR 184.710

Each ring life buoy on a vessel must be approved IAW 160.050. Buoy with light is no longer servicable. Additionally, grab lines and light lanyard are not properly arranged. 46 CFR 180.70

Each ring life buoy on a vessel must be marked with type II retroreflective material approved IAW 164.018 and as specified by IMO resolution A.658(16).02. Outboard ring buoys lacked 3/4 of required retroreflective material. 46 CFR 185.604(i)

Vsl is not broadcasting dimension information (AIS Antenna Position ABCD values equals 0) and Vsl is not broadcasting static draft information

Provide eye wash, aspirin, and iodine applicators for first aid kit.

Prove proper operation at 50 GPM of bilge pump.

Repair port main diesel engine to satisfaction of USCG marine Inspector and provide faxed copy said tech report to USCG.

Obtain current FCC Safety Radio Certificate. Vessel operation restricted to no more than 1000 feet from shore until this requirement is met.
