PSCO observed that manufacturers maintenance instructions for the ships fire extinguishers were not on board. The safety management system for the ship requires that these instructions be available on board and used to maintain the fire extinguishers. The crew was not using these manufactures instructions to complete maintenance or inspections to the extinguishers. 74SOLAS(20)IX/3.1 - 50a,c

Fire doors in main vertical zone bulkheads, galley boundaries, and stairway enclosures other than power-operated watertight doors and those which normally locked shall be self-closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. PSCO observed 02 self-closing doors at galley and 01 on deck above it not closing.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed inoperable medicinal fridge in ship's hospital. Fridge is unable to maintain medicine at a temperature where they will become ineffective, specifically, the Insulin stored within.

Every ship of 400 gross tonnage and above other than an oil tanker shall be provided with an Oil Record Book Part I (Machinery Space Operations). The ORB shall be in the form specified in appendix III to this Annex. PSCO observed two entries incorrect. The entries are related to OWS operations. Both times code 15.3 was used instead of 15.1 since means of discharge was through 15 ppm equipment.

Oil filtering equipment referred to in paragraph 1 of this regulation shall be of a design approved by the Administration and shall be such as to ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15ppm. PSCO found a modification to the OWS' sample line. This altered the piping system and method to test 15 ppm alarm. This is not approved by Class nor is it IAW OWS' operations manual.
