The craft and equipment shall be maintained to the satisfaction of the Administration. Vessel had work vests on board that were not annually serviced. HSCC 2000 19.2

An owner or operator of a ship to which this subpart applies must review the SOPEP annually and submit a letter to Commandant (CG-MER) certifying that the review has been completed. Vessel has not completed their annual review of the SOPEP. 33 CFR 151.28

Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm setpoints, the procedure to be followed, and the expected test result. Generator low l/o set point per procedure is 29 psi. Cat service report stated it was set at 23 psi which is not per approved test procedure. 46CFR61.40

Tests must verify that automated vital systems are designed, constructed, and operate in accordance with all applicable requirements of part 62 of this subchapter. Test techniques must not simulate monitored system conditions by mis-adjustment, artificial signals, improper wiring, tampering, or revision of the system unless the test would damage equipment or endanger personnel. In the latter case, the use of a synthesized signal or condition applied to the sensor is acceptable if test equipment is maintained in good working order and is periodically calibrated to the satisfaction of the Officer in Charge, Marine Inspection. Other test techniques must be approved by the Commanding Officer, Marine Safety Center. Test for requiring power reduction as failsafe for MDE not being conducted per approved periodic safety test procedure. Tests are being simulated on SSDG and not per approved periodic safety test procedure. Complete test per procedure or resubmit for approval how OEM's are conducting tests.

Vessel is restricted to carrying crew only and may not carry passengers until all of the primary lifesaving equipment listed on the "RECORD OF EQUIPMENT FOR HIGH-SPEED CRAFT SAFETY CERTIFICATE" is in working order and ready for immediate use. ALAKAI is permitted to operate with crew only while carrying a single MES (stbd-aft), and a spare 36-person liferaft.

Crack in pre lube pump piping on STBD ME. Replace with new pipe section.

The vessel is currently broadcasting (1) Improper Name. The HSV before the name and Space must be removed so that the vessel name is only”ALAKAI”.

Contact Sector Northern New England prior to departing Yarmouth, N.S. and after completing temporary repairs.

Complete permanent repairs to port hull damage in void #4. Repairs shall be acceptable to USCG and Class Society. Any failure in the existing temporary repairs shall be immediately communicated to the USCG and Class Society. If the vessel is U/W it may complete the voyage. If the vessel is moored it shall remain moored until the failure is assessed by USCG and Class Society.

Grind out & weld lower forward corner on the aft side of frame 14. Small leak to be repaired.

Show proper operation of remote operation to bus tie relay.

Renew liferings on port and starboard bridge wings with properly weighted rings that are suitable for quick release operation.

Reduce the total persons allowed on board to 545 total persons to meet 110% liferaft requirement with six at 100 man rafts.

Complete the remaining requirements of USCG Marine Safety Center letter 16710/P013651 SER# E1-0903143 Dated November 20, 2009 concerning the machinery space water mist system water supply arrangements.

Provide automation test procedures for the machinery space water mist fire suppression system.

The automation test procedures for the ships service diesel generators do not include specific alarm set points per 46 CFR 61.40-10(a). submit to [name withheld] updated test procedures that include specific alarm set points.

Demonstrate proper operation of the ventilation supply dampers in the starboard main engine space.

Replace the pressure gage for the starboard main engine room air receiver and prove proper operation.

Demonstrate proper operation of the oil level float sensors for the #2 and #4 main engines. Establish and maintain an alternate monitoring schedule for the sensors until operation is proven.

Perform automation tests on #1 main engine.

Machinery space water mist system does not meet the Marine Safety Center (MSC) requirement for a water supply arrangement as per MSC letters E1-0703317 dated, 29 Oct 2007, E1-0900483 dated, 26 Feb 2009 and E0-0900621 dated, 04 Mar 2009. Address and make modifications as required by the aforementioned MSC letters.

Remove unapproved PVC piping in voids 15 and 16 for the black and grey water discharge, sludge oil, and freshwater fill as per MSC Letter E1-0801649 Dated Jun 09, 2008 and install approved piping.

Install non-return valve in fire main system piping in way of generator and tunnel bulkhead as per sister vessel HUAKAI (ON: 1215902) approved drawing 615-652-000 Rev B, Detail 1 for both port and starboard hulls.

The machinery space water mist system aboard the vessel does not meet the MSC requirement for a water supply arrangement as per MSC letters E1-0703317 dated 29Oct07, E1-0900483 dated 26Feb09 and E0-0900621 dated 4Mar09. Submit plans to the MSC addressing requirement to install a 2655 gallon fresh water supply NLT 7Apr09. Upon approval by MSC, install the fresh water supply IAW plans by 07Jun09.

A 5 minute supply of water from the onboard fresh water tank shall be reserved and available exclusively for the mist system.

Make mist system transfer to sea water fully automatic with no delay and prove all above functions to the satisfaction of the OCMI.

Provide means to protect personnel from excessive heating of forward bulkheads in jet room. Address potential for structural problems with this bulkhead due to excessive heat. Temperature of bulkhead during sea trials measured up to 190 degrees Celsius during full power endurance trial.

Complete proper fire drill in accordance with HSC Code 18.5, MSM Vol. II/22.C.7 and NVIC 6-91 - to the satisfaction of the attending USCG Marine Inspector.

The manual operating positions for automatic fire dampers in ventilation ducts passing through main vertical zone bulkheads shall be identified by red day light-reflecting letters at least 1/2 inch high ‘‘VENTILATION FIRE DAMPER.’’ In addition, the open and closed positions shall be similarly marked.

Submit periodic safety test procedures for review & approval to USCG [name withheld] prior to 24 August 2007. Demonstrate proper operation of all engine safety devices to protect against overspeed, loss of lube oil pressure, loss of cooling medium, high temperature, malfunction of moving parts, and overload per HSC Code 9.2.2 and to the discretion of [name withheld].

Provide evidence of Administration approval for daylight signaling lamp per HSC Code 13.17.1, but no later than 24 July 2007.

Provide evidence of Administration approval for night vision equipment per HSC Code 13.17.1, but no later that 24 July 2007.

Demonstrate adequate cruise performance & testing in all sea conditions required by HSC Code and Annex 9 Section 3.

In addition to the emergency lighting required by 12.7.3.1, 12.7.4.1, and 12.7.5.1, on every craft with RORO spaces, all passenger public spaces and alleyways shall be provided with supplementary electric lighting that can operate for at least 3 hours when all other sources of electrical power have failed and under any condition of heel. Prove proper operation of lighting required by HSC Code 12.

Provide Coast Guard approved dead-ship start procedures to insure a safe & orderly manual restoration of the ship's service electrical power in the event of a failure of a blackout start system as per MSC Letter Serial No. E2-0603840 dated December 29, 2006. Satisfy any outstanding comments from the plan review process.

Provide USCG Approved Vessel/Coast Guard SAR Plan in accordance with SOLAS 74/78 V/15.

Provide documentation & procedures that demonstrate compliance for the information on passengers as per HSC Code 18.2.5.

Provide approved craft maintenance & servicing manual/system as required by HSC Code Chapter 18 addressing items contained in 18.2.4

Demonstrate proper local and remote operation of all fire doors in fire resisting divisions per HSC Code 7.9.3. Doors at tops of engine room stairtowers do not self close, and are very difficult to manually close when all engineroom ventilation systems are running.

Provide approved training manual to include all of the information required by HSC Code 18.2.3 for locations required under HSC Code 8.4.5.

Provide means to reduce ambient noise level on aft passenger weather deck to below 75d(B)A per HSC Code 4.10.1, or provide information proving that existing dBA levels do not exceed allowable time weighted averages when calculated & based on length of actual voyage legs.

Complete proper evacuation drill in accordance with HSC Code 4.8 to the satisfaction of the attending USCG Marine Inspector.

All exterior seating on the aft weather deck shall meet the prescriptive requirements and design standards of HSC Code 4.5 and HSC Code Annex 10.

Provide MSC approved procedures for conducting HSC Code Annex 9 testing if intentions are to conduct this testing during delivery voyage.
