The craft and equipment shall be maintained to the satisfaction of the Administration. Vessel had work vests on board that were not annually serviced. HSCC 2000 19.2

An owner or operator of a ship to which this subpart applies must review the SOPEP annually and submit a letter to Commandant (CG-MER) certifying that the review has been completed. Vessel has not completed their annual review of the SOPEP. 33 CFR 151.28

Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm setpoints, the procedure to be followed, and the expected test result. Generator low l/o set point per procedure is 29 psi. Cat service report stated it was set at 23 psi which is not per approved test procedure. 46CFR61.40

Tests must verify that automated vital systems are designed, constructed, and operate in accordance with all applicable requirements of part 62 of this subchapter. Test techniques must not simulate monitored system conditions by mis-adjustment, artificial signals, improper wiring, tampering, or revision of the system unless the test would damage equipment or endanger personnel. In the latter case, the use of a synthesized signal or condition applied to the sensor is acceptable if test equipment is maintained in good working order and is periodically calibrated to the satisfaction of the Officer in Charge, Marine Inspection. Other test techniques must be approved by the Commanding Officer, Marine Safety Center. Test for requiring power reduction as failsafe for MDE not being conducted per approved periodic safety test procedure. Tests are being simulated on SSDG and not per approved periodic safety test procedure. Complete test per procedure or resubmit for approval how OEM's are conducting tests.

Vessel is restricted to carrying crew only and may not carry passengers until all of the primary lifesaving equipment listed on the "RECORD OF EQUIPMENT FOR HIGH-SPEED CRAFT SAFETY CERTIFICATE" is in working order and ready for immediate use. ALAKAI is permitted to operate with crew only while carrying a single MES (stbd-aft), and a spare 36-person liferaft.

Crack in pre lube pump piping on STBD ME. Replace with new pipe section.

The vessel is currently broadcasting (1) Improper Name. The HSV before the name and Space must be removed so that the vessel name is only”ALAKAI”.

Contact Sector Northern New England prior to departing Yarmouth, N.S. and after completing temporary repairs.

Complete permanent repairs to port hull damage in void #4. Repairs shall be acceptable to USCG and Class Society. Any failure in the existing temporary repairs shall be immediately communicated to the USCG and Class Society. If the vessel is U/W it may complete the voyage. If the vessel is moored it shall remain moored until the failure is assessed by USCG and Class Society.
